About SMOFlipid
Overview
SMOFlipid is a lipid emulsion containing soybean oil, medium-chain triglycerides, olive oil and fish oil, given by infusion into a vein. It provides calories and essential fatty acids for adults and children, including full-term and premature newborns, when nutrition by mouth or feeding tube is not possible, is insufficient or should not be used. The body uses its fatty acids for energy and to support the structure and function of cell membranes.

Safety information
Side effects
Common side effects reported in adult clinical trials were nausea, vomiting and high blood sugar. In children, common side effects were anemia, vomiting, increased levels of the enzyme gamma-glutamyltransferase (GGT), and infections acquired in hospital. Each occurred in at least 5% of patients in the relevant trials. Serious reactions that warrant prompt medical attention include allergic reactions, infections, liver disease and other liver or bile-related disorders, fat overload syndrome, refeeding syndrome, high blood triglycerides, aluminum toxicity and essential fatty acid deficiency. The infusion must be stopped if an allergic reaction occurs. Blood tests and monitoring are needed to check for several of these problems. Rapid infusion of intravenous lipid emulsions in newborns and infants has been associated with severe breathing distress, excess acid in the blood and death. The risk of liver disease is higher with intravenous nutrition lasting more than two weeks, especially in premature newborns. Aluminum toxicity is more likely in patients with impaired kidney function, including premature newborns.

Before taking this medicine
SMOFlipid must not be used in anyone with a known allergy to fish, egg, soybean, peanut or any of its ingredients. It must also not be used in people with severe disorders of fat metabolism involving blood triglyceride levels above 1,000 mg/dL. Special caution and monitoring are needed in newborns and infants: rapid infusion has been linked to acute breathing difficulties, excessive acid in the blood and death. Intravenous nutrition lasting more than two weeks increases the risk of liver disease, especially in premature newborns. Liver blood tests should be monitored. Monitoring is also needed for allergic reactions, infections, fat overload syndrome, refeeding syndrome, high triglycerides and essential fatty acid deficiency. The infusion must be stopped if an allergic reaction occurs. People with impaired kidney function, including premature newborns, are at increased risk of aluminum toxicity.
Interactions
SMOFlipid contains vitamin K1 from soybean and olive oils, which may reduce the anticoagulant effect of vitamin K antagonists such as warfarin. Tell your doctor if you take warfarin or another vitamin K antagonist. If you receive SMOFlipid with warfarin, more frequent blood tests are needed to monitor its anticoagulant effect.

Storage
Store in the original carton below 25°C, protected from light, unless the pack states refrigeration. Do not freeze. Keep all medicines out of the sight and reach of children.
