About Zokinvy
Overview
Zokinvy (lonafarnib) is a prescription capsule for patients aged 12 months and older with a body surface area of at least 0.39 m². It is used to reduce the risk of death in Hutchinson-Gilford Progeria Syndrome and to treat certain processing-deficient progeroid laminopathies involving a heterozygous LMNA mutation with progerin-like protein accumulation, or homozygous or compound heterozygous ZMPSTE24 mutations. It is not indicated for other progeroid syndromes or processing-proficient progeroid laminopathies. It works by blocking an enzyme called farnesyltransferase, helping prevent progerin and progerin-like proteins from building up in the inner membrane surrounding the cell’s nucleus.
What it treats
For patients aged 12 months and older with a body surface area of at least 0.39 m²: - Hutchinson-Gilford Progeria Syndrome (HGPS), to reduce the risk of death. - Processing-deficient progeroid laminopathies with either a heterozygous LMNA mutation causing progerin-like protein accumulation, or homozygous or compound heterozygous ZMPSTE24 mutations. Not indicated for other progeroid syndromes or processing-proficient progeroid laminopathies.
Safety information
Side effects
Common side effects include vomiting, diarrhea, nausea, decreased appetite, tiredness, abdominal pain, muscle and joint pain, weight loss, headache, cough, infections, upper respiratory tract infections, and high blood pressure. Common laboratory changes include electrolyte imbalances, reduced blood bicarbonate, reduced blood cell production, and increased liver enzymes. Zokinvy can prolong the QTc interval, a change in the heart’s electrical activity. It should be avoided in people with a slow heartbeat that causes symptoms, low potassium or magnesium levels, or those taking other medicines that prolong the QTc interval. Other drug interactions can increase side effects or reduce treatment effectiveness, so your doctor should review your medicines before and during treatment. Contact your doctor if you notice any new vision changes. Low-light vision decline occurred in monkeys, and kidney toxicity occurred in rats. Regular eye examinations and kidney function checks are recommended. Your doctor should also monitor electrolytes, blood cell counts, and liver enzymes. Zokinvy can harm an unborn baby. Contact your doctor if you become pregnant or think you may be pregnant, and use effective contraception if you could become pregnant. Animal studies also found impaired fertility and damage to male reproductive organs; discuss potential fertility risks with your doctor.

Before taking this medicine
Do not take Zokinvy with strong CYP3A inhibitors, strong or moderate CYP3A inducers, midazolam, lovastatin, simvastatin or atorvastatin. Your prescriber should review your other medicines before and during treatment because interactions may reduce effectiveness or increase adverse reactions. Zokinvy can prolong the QTc interval, a measure of the heart’s electrical activity. Avoid use if you have a slow heartbeat that causes symptoms, low blood potassium or magnesium, or take other medicines known to prolong the QTc interval. Treatment requires monitoring of electrolytes, blood cell counts, liver enzymes and kidney function. Regular eye examinations are also needed, with an examination if any new vision changes occur. Kidney damage and reduced low-light vision were seen in animal studies. Zokinvy can harm an unborn baby. Females who could become pregnant should use effective contraception. Animal studies also found impaired fertility and harm to male reproductive organs; discuss these risks with your prescriber.
Interactions
