About Signifor

Overview

Signifor contains pasireotide, as pasireotide diaspartate, and is an injection given under the skin. It is used to treat adults with Cushing’s disease when pituitary surgery is not an option or has not been curative. It works by activating somatostatin receptors to reduce the release of ACTH, a hormone that stimulates cortisol production, thereby lowering cortisol secretion.

Side effects

Common side effects include diarrhea, nausea, high blood sugar, gallstones, headache, abdominal pain, tiredness and diabetes. Each was reported in at least 20% of patients. Contact your doctor about serious side effects, including low cortisol levels, high blood sugar or diabetes, a slow heartbeat or changes in the heart’s electrical rhythm (QT prolongation), abnormal liver tests, gallstone complications and low pituitary hormone levels. Tell your doctor if you develop new or worsening fatty stools, changes in stool color, loose stools, abdominal bloating or weight loss. These may indicate poor absorption of dietary fats and need assessment for reduced pancreatic enzyme function. Blood sugar requires close monitoring when treatment starts. Heart rhythm and liver tests are recommended before and during treatment, along with periodic checks for gallstones.

Before taking this medicine

The label lists no contraindications for Signifor. However, it requires caution and monitoring because it may cause low cortisol levels, high blood sugar or diabetes, a slow heartbeat, QT prolongation (a change in the heart’s electrical activity), liver test abnormalities, and gallstones or their complications. Blood glucose should be monitored closely when treatment starts, and diabetes treatment may need to be started or adjusted. An ECG and liver tests are recommended before and during treatment, along with periodic monitoring for gallstones. Low cortisol may require treatment changes, and Signifor should be discontinued if gallstone complications are suspected. New or worsening fatty stools, stool discoloration, loose stools, abdominal bloating, or weight loss should be assessed for possible pancreatic enzyme insufficiency. Premenopausal women should be advised about the potential for unintended pregnancy. Safety and effectiveness in children under 18 years have not been established.

Interactions

Tell your doctor if you take medicines that prolong the QT interval (affect the heart’s electrical rhythm), as caution is needed if you are at significant risk of QT prolongation. Also mention cyclosporine, which may require additional monitoring, and bromocriptine, whose dose your doctor may need to reduce.
