About Sensipar

Overview

Sensipar is a prescription medicine containing cinacalcet, supplied as tablets. It treats secondary hyperparathyroidism in adults with chronic kidney disease who are on dialysis. It also treats high blood calcium in adults with parathyroid cancer, or with primary hyperparathyroidism when their calcium levels warrant surgery to remove the parathyroid glands but they cannot undergo it. It works by making calcium-sensing receptors in the parathyroid glands more sensitive to calcium, lowering parathyroid hormone levels and, in turn, blood calcium levels.

What it treats

- Secondary hyperparathyroidism in adults with chronic kidney disease who are on dialysis. - High blood calcium in adults with parathyroid cancer. - High blood calcium in adults with primary hyperparathyroidism who need surgery to remove the parathyroid glands based on their calcium levels but cannot undergo the surgery.

Safety information

Side effects

The most common side effects of Sensipar are nausea and vomiting, each reported in at least 25% of patients. Sensipar can cause low blood calcium, which may be life-threatening or fatal. It can affect the heart’s electrical activity, cause abnormal heart rhythms or low blood pressure, worsen heart failure, and increase the risk of seizures. Contact a doctor immediately if you have a seizure, an abnormal heartbeat or worsening heart failure. Blood calcium must be monitored carefully during treatment. Excessive suppression of parathyroid hormone can lead to adynamic bone disease, a condition in which bone turnover becomes too low. People with moderate or severe liver impairment have increased exposure to cinacalcet and need close monitoring of calcium, phosphorus and parathyroid hormone levels throughout treatment.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not start Sensipar if your blood calcium is below the normal range. It is not indicated for children; a death was reported in a child with severely low calcium during a clinical trial. Blood calcium must be monitored carefully during treatment. Low calcium can be life-threatening and may cause heart rhythm changes, increase the risk of seizures, lower blood pressure or worsen heart failure. Caution and close monitoring of calcium, phosphorus and parathyroid hormone levels are needed if you have moderate or severe liver impairment. Excessive suppression of parathyroid hormone may lead to adynamic bone disease, a condition in which bone turnover is reduced. During pregnancy, Sensipar should be used only if the potential benefit justifies the potential risk to the fetus.

Interactions

Strong CYP3A4 inhibitors, such as ketoconazole, can increase cinacalcet levels in the blood. Your doctor may need to adjust your treatment and monitor parathyroid hormone (PTH), phosphorus and calcium levels. Cinacalcet strongly blocks CYP2D6, an enzyme that breaks down certain medicines. It can increase exposure to medicines such as desipramine and amitriptyline. Tell your doctor if you take these medicines or other medicines mainly broken down by CYP2D6, as their doses may need adjustment.

Dosing

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
