About Ravicti
Overview
Ravicti (glycerol phenylbutyrate) is a nitrogen-binding medicine used for long-term management of urea cycle disorders when dietary protein restriction and/or amino acid supplements alone are not enough. It must be used with dietary protein restriction and, in some cases, dietary supplements. It helps the body remove waste nitrogen through the urine by providing an alternative pathway for its removal. Ravicti is not for treating sudden episodes of high blood ammonia, which require faster-acting treatment.
What it treats
- Long-term management of urea cycle disorders when dietary protein restriction and/or amino acid supplements alone are not enough. Used alongside dietary protein restriction and, in some cases, dietary supplements. Not for treating acute high blood ammonia levels. Safety and effectiveness for N-acetylglutamate synthase (NAGS) deficiency have not been established.
Safety information
Side effects
Common side effects in adults include diarrhea, gas and headache. Ravicti may cause serious toxicity affecting the nervous system. Contact your doctor if you suspect nervous system side effects. If you have reduced pancreatic function or difficulty absorbing nutrients from the intestines, your doctor should monitor your ammonia levels closely.
Before taking this medicine
Do not take Ravicti if you have a known allergy to phenylbutyrate. Signs of an allergic reaction include wheezing, difficulty breathing, coughing, low blood pressure, flushing, nausea and rash. Ravicti requires caution in people with pancreatic insufficiency or intestinal malabsorption; ammonia levels should be monitored closely. Phenylacetate, the active substance produced from Ravicti, may be toxic to the nervous system. Contact your prescriber if symptoms of nervous system toxicity occur. Breastfeeding is not recommended during treatment.
Interactions
Tell your doctor if you take corticosteroids, valproic acid or haloperidol, as these may raise blood ammonia levels and require close monitoring. Probenecid may affect how the kidneys remove substances produced when Ravicti is broken down. Ravicti may reduce the effects of certain medicines, including alfentanil, quinidine, cyclosporine and midazolam. Your doctor may need to monitor how well these medicines are working.

Dosing
Dosing is set by your prescriber. Each health care professional may prescribe medication differently according to a patient's medical history and condition, so follow the directions on your prescription label and speak to your doctor or pharmacist before changing anything.
Storage
Store below 25°C, upright and tightly closed, away from direct light. Check the label for how long it may be used after opening. Keep all medicines out of the sight and reach of children.
