About Cerdelga

Overview

Cerdelga (eliglustat) is a prescription medicine supplied as capsules. It is used for the long-term treatment of adults with Gaucher disease type 1 whose CYP2D6 test identifies them as extensive, intermediate or poor metabolizers, meaning how their body processes the medicine. It works by reducing the production of glucosylceramide, a fatty substance that builds up in organs in Gaucher disease.

What it treats

Long-term treatment of Gaucher disease type 1 in adults confirmed by an FDA-cleared test to be CYP2D6 extensive, intermediate or poor metabolizers.

Side effects

Common side effects include tiredness, headache, nausea, diarrhea, back pain, pain in the arms or legs, and upper abdominal pain. Serious risks include changes in the heart’s electrical activity (ECG changes) and potentially abnormal heart rhythms. Contact your doctor if you develop an abnormal heart rhythm. Cerdelga is not recommended for people with pre-existing heart disease or long QT syndrome, or those taking Class IA or Class III medicines for abnormal heart rhythms. Tell your doctor if any of these apply to you.

Before taking this medicine

Cerdelga must not be taken with certain medicines or in certain liver conditions because of the risk of abnormal heart rhythms. The restrictions depend on your CYP2D6 metabolizer status, how your body processes the medicine: • Extensive metabolizers: Do not take Cerdelga if you have moderate or severe liver impairment; if you have mild liver impairment and take a strong or moderate CYP2D6 inhibitor; or if you take both a strong or moderate CYP2D6 inhibitor and a strong or moderate CYP3A inhibitor. • Intermediate metabolizers: Do not take Cerdelga if you have any liver impairment; take a strong CYP3A inhibitor; or take both a strong or moderate CYP2D6 inhibitor and a strong or moderate CYP3A inhibitor. • Poor metabolizers: Do not take Cerdelga if you have any liver impairment or take a strong CYP3A inhibitor. Cerdelga is not recommended for people with pre-existing heart disease or long QT syndrome, or those taking Class IA or Class III medicines for abnormal heart rhythms. It can affect the heart’s electrical conduction and increase the risk of abnormal heart rhythms.

Interactions

Eliglustat interacts with medicines that affect the liver enzymes CYP2D6 and CYP3A. These interactions can raise eliglustat levels and increase the risk of heart rhythm problems. Examples include clarithromycin, ketoconazole, itraconazole, ritonavir, fluoxetine and paroxetine. Some combinations must not be used; restrictions depend on how your body processes eliglustat and on your liver function. Rifampicin, carbamazepine, phenytoin, phenobarbital and the herbal supplement St John’s wort can lower eliglustat levels and reduce its effect. Avoid grapefruit and grapefruit juice, which can increase eliglustat levels. Tell your doctor if you take: • Heart rhythm medicines, such as amiodarone, quinidine, sotalol, flecainide or propafenone. • Digoxin or metoprolol, as eliglustat can increase their levels and may require changes to treatment or monitoring. Have your doctor or pharmacist check all prescription medicines, over-the-counter products and supplements before starting or stopping them while taking Cerdelga.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
