About Zestril

Overview

Zestril tablets contain lisinopril, an angiotensin-converting enzyme (ACE) inhibitor. It treats high blood pressure in adults and children aged 6 years and older and is used alongside other treatment to reduce symptoms of systolic heart failure. It is also used to reduce the risk of death in patients whose circulation is stable within 24 hours of an acute heart attack. It works by reducing the production of angiotensin II, a substance that narrows blood vessels, helping to lower blood pressure.

What it treats

- High blood pressure in adults and children aged 6 years and older. - Systolic heart failure, as an add-on treatment to reduce signs and symptoms. - Acute heart attack, to reduce the risk of death in patients with stable circulation when treatment begins within 24 hours.

Side effects

Common side effects of Zestril (lisinopril) include headache, dizziness and cough in people treated for high blood pressure. Low blood pressure and chest pain were commonly reported in people treated for heart failure, and low blood pressure was common after a heart attack. Other reported effects include tiredness, weakness, diarrhea, constipation and dry mouth. Serious side effects need medical attention: • Stop taking Zestril and seek emergency help for swelling of the face, lips, tongue or throat, or difficulty breathing. This reaction, called angioedema, can occur at any time during treatment and can be fatal. Swelling of the intestines can cause abdominal pain, with or without nausea or vomiting. • Contact a doctor promptly for fainting, severe dizziness or reduced urination. Zestril can cause severe low blood pressure and kidney problems, including acute kidney failure. • Contact a doctor promptly for yellowing of the skin or eyes. Liver injury associated with ACE inhibitors can progress to liver failure and death. • Seek urgent medical attention for a severe skin reaction. Stevens–Johnson syndrome and toxic epidermal necrolysis have been reported. Zestril can also raise blood potassium levels. Kidney function and potassium levels should be checked periodically during treatment. Other serious reported effects include inflammation of the pancreas and rare blood disorders affecting red blood cells, white blood cells or platelets. Zestril can harm or cause the death of an unborn baby. If pregnancy is detected, stop taking Zestril as soon as possible and contact your doctor.

When to call your doctor

- Seek immediate medical attention for swelling of the face, lips, tongue or throat, or difficulty breathing. - Get urgent medical help for severe abdominal pain, with or without nausea or vomiting. - Contact your doctor immediately for yellowing of the skin or eyes. - Get urgent medical help for fainting, severe dizziness or passing very little urine. - Seek immediate medical attention for a severe rash with blistering or peeling skin.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Zestril if you have had angioedema (swelling beneath the skin) or an allergic reaction related to a previous ACE inhibitor, or if you have hereditary angioedema or angioedema with no known cause. Do not take it with a neprilysin inhibitor such as sacubitril, or within 36 hours of switching to or from sacubitril/valsartan. If you have diabetes, do not take Zestril with aliskiren. Zestril can harm an unborn baby. If pregnancy is detected, stop taking it as soon as possible and contact your prescriber. Do not breastfeed while taking Zestril. Extra caution and monitoring are needed if you have kidney disease, narrowing of the arteries supplying the kidneys, heart failure, a recent heart attack, low blood pressure, reduced fluid or salt levels, heart or brain blood vessel disease, severe aortic valve narrowing, or hypertrophic cardiomyopathy. Zestril should be avoided after an acute heart attack if blood circulation is unstable. Kidney function, blood pressure and potassium levels need monitoring. Tell your prescriber if you take diuretics, potassium supplements, potassium-containing salt substitutes, or mTOR inhibitors such as sirolimus, everolimus or temsirolimus. Diabetes and kidney impairment increase the risk of high potassium. mTOR inhibitors increase the risk of angioedema. Tell your healthcare team before surgery or anesthesia, dialysis, LDL apheresis with dextran sulfate, or insect-venom desensitization treatment. These situations can increase the risk of low blood pressure or severe allergic-like reactions. Angioedema can occur at any time during treatment and may be life-threatening. Stop Zestril and seek urgent medical help for swelling of the face, lips, tongue or throat, or difficulty breathing. Unexplained abdominal pain, with or without nausea or vomiting, may be a sign of intestinal angioedema. A previous history of angioedema unrelated to ACE inhibitors also increases the risk; angioedema occurs more often in Black patients. Stop Zestril and seek medical care if you develop jaundice (yellowing of the skin or eyes). ACE inhibitors have been associated with severe, sometimes fatal liver injury.

Interactions

Diuretics (“water pills”) can cause an excessive fall in blood pressure when Zestril is started. Potassium-sparing diuretics, such as spironolactone, amiloride and triamterene, can also raise blood potassium; frequent potassium checks are needed if used together. Insulin and other diabetes medicines may lower blood sugar more when taken with Zestril, increasing the risk of low blood sugar. Non-steroidal anti-inflammatory drugs (NSAIDs), including COX-2 inhibitors, may reduce Zestril’s blood-pressure-lowering effect and worsen kidney function, potentially causing acute kidney failure. This risk is greater in older people, those with low fluid levels and those with existing kidney problems. Kidney function should be monitored during combined use. Combining Zestril with other ACE inhibitors, angiotensin receptor blockers (ARBs) or aliskiren generally should be avoided because it increases the risk of low blood pressure, high potassium and kidney problems. Aliskiren must not be used with Zestril in people with diabetes and should be avoided in people with impaired kidney function. Zestril can increase the risk of lithium toxicity. Blood lithium levels should be monitored if these medicines are used together. Injectable gold treatment (sodium aurothiomalate) has rarely caused facial flushing, nausea, vomiting and low blood pressure when used with Zestril. mTOR inhibitors, such as temsirolimus, sirolimus and everolimus, and neprilysin inhibitors may increase the risk of angioedema, a swelling reaction, when taken with Zestril.
