Increased risk of serious bacterial, fungal, viral, and opportunistic infections, including tuberculosis (TB), leading to hospitalization or death. Interrupt XELJANZ/XELJANZ XR treatment if serious infection occurs until the infection is controlled. Test for latent TB before and during therapy; treat latent TB prior to use. Monitor all patients for active TB during treatment, even patients with initial negative latent TB test.
Higher rate of all-cause mortality, including sudden cardiovascular (CV) death with XELJANZ vs. TNF blockers in rheumatoid arthritis (RA) patients.

About Xeljanz
Overview
Xeljanz tablets contain tofacitinib citrate, a Janus kinase (JAK) inhibitor. They are used in adults with moderately to severely active rheumatoid arthritis or ulcerative colitis, active psoriatic arthritis, or active ankylosing spondylitis when one or more TNF blockers have not worked well enough or could not be tolerated. Xeljanz immediate-release tablets are also used in children aged 2 years and older with active psoriatic arthritis or active polyarticular course juvenile idiopathic arthritis under the same conditions. Tofacitinib works by blocking JAK enzymes that carry signals involved in immune cell activity.
What it treats
For adults who have not responded adequately to, or cannot tolerate, one or more TNF blockers: • Moderately to severely active rheumatoid arthritis. • Active psoriatic arthritis. • Active ankylosing spondylitis. • Moderately to severely active ulcerative colitis. Xeljanz tablets are also used in children aged 2 years and older who have not responded adequately to, or cannot tolerate, one or more TNF blockers: • Active psoriatic arthritis. • Active polyarticular-course juvenile idiopathic arthritis.
Safety information
Side effects
Common side effects include upper respiratory tract infections, cold-like symptoms, diarrhoea and headache. In adults treated for ulcerative colitis, common side effects also include raised cholesterol, increased blood creatine phosphokinase, rash and shingles. Common side effects in children with polyarticular-course juvenile idiopathic arthritis are consistent with those reported in adults with rheumatoid arthritis. Serious risks include serious infections, cancer and disorders involving abnormal growth of lymphatic cells, major cardiovascular events, blood clots, holes in the stomach or intestines, and allergic reactions. The label also warns of an increased risk of death. Contact a doctor promptly if you develop a serious infection, new abdominal symptoms, or symptoms of an allergic reaction, cardiovascular event or blood clot. Low blood sugar can occur in people with diabetes. Tell your healthcare provider if you develop symptoms of low blood sugar; more frequent blood glucose checks may be needed. Treatment can also affect white blood cell counts, haemoglobin, liver enzymes and blood fats, so laboratory monitoring is recommended.

Before taking this medicine
The label lists no formal contraindications. However, Xeljanz should be avoided during an active serious infection, including a serious infection limited to one area of the body. People at increased risk of a tear in the stomach or intestine need prompt medical assessment if new abdominal symptoms develop. People with diabetes may need more frequent blood glucose checks and should tell their healthcare provider if they develop signs or symptoms of low blood sugar. Laboratory monitoring is recommended because treatment may change certain white blood cell counts, haemoglobin, liver enzymes and blood fats. Avoid live vaccines during treatment. Do not breastfeed while taking this medicine.
Interactions
Ketoconazole and fluconazole can increase the amount of tofacitinib in the body. Your prescriber may need to adjust your dose if you take these medicines. Rifampin can lower the amount of tofacitinib in the body and may reduce its effect. Taking it with Xeljanz is not recommended. Taking Xeljanz with biologic disease-modifying antirheumatic drugs (DMARDs) or potent immune-suppressing medicines, such as azathioprine, tacrolimus or cyclosporine, is not recommended because of the risk of added immune suppression. Tell your doctor if you take any of these medicines.

Storage
Store below 25°C in a dry place, in the original blister until you take a dose, away from direct light and moisture. Keep all medicines out of the sight and reach of children.
