SUICIDALITY AND ANTIDEPRESSANT DRUGS

Antidepressants increased the risk of suicidal thoughts and behavior in children, adolescents, and young adults in short-term trials. These trials did not show an increase in the risk of suicidal thoughts and behavior with antidepressant use in subjects over age 24; there was a reduction in risk with antidepressant use in subjects aged 65 and older.

In patients of all ages who are started on antidepressant therapy, monitor closely for worsening, and for emergence of suicidal thoughts and behaviors. Advise families and caregivers of the need for close observation and communication with the prescriber.

About Wellbutrin SR

Overview

Wellbutrin SR is an antidepressant containing bupropion hydrochloride, supplied as sustained-release tablets. It is used to treat major depressive disorder. Its exact antidepressant action is not known, but it is thought to work by reducing the reabsorption of the brain chemicals norepinephrine and dopamine.

Side effects

Common side effects include headache, dry mouth, nausea, trouble sleeping, dizziness, sore throat, constipation, agitation, anxiety, abdominal pain, ringing in the ears, shaking, palpitations, muscle pain, sweating, rash and loss of appetite. Serious side effects include seizures, increased blood pressure, mania or hypomania, psychosis, angle-closure glaucoma and allergic reactions. Contact a doctor if these occur. Stop taking Wellbutrin SR if a seizure occurs. Blood pressure should be checked before treatment and periodically during treatment. Suicidal thoughts and behaviours are a serious risk, particularly in adolescents and young adults. Seek immediate medical help for suicidal thoughts or behaviour. Serious changes in mood, thinking or behaviour have also been reported in people taking bupropion while trying to quit smoking. These include depression, mania, hallucinations, paranoia, delusions, aggression, hostility, agitation, anxiety, panic and thoughts of harming others or themselves. If these occur while trying to quit smoking, stop taking bupropion and contact a healthcare professional immediately.

When to call your doctor

• Seek medical attention immediately for thoughts of suicide, self-harm or harming someone else, or worsening depression. • Stop taking Wellbutrin SR and seek medical attention immediately if you have a seizure. • Contact your healthcare professional promptly if you see or hear things that are not there, feel intensely suspicious, or believe things that are not true. • Contact your healthcare professional promptly for an unusually elevated mood or a marked increase in energy and activity. • If you are taking bupropion while trying to quit smoking, stop taking it and contact your healthcare professional if you develop mood changes, aggression, agitation, anxiety or panic.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Wellbutrin SR if you have a seizure disorder, have or have ever had bulimia or anorexia nervosa, or are allergic to bupropion or any other ingredient. It must not be used while abruptly stopping alcohol, benzodiazepines, barbiturates or antiepileptic medicines. Do not take Wellbutrin SR with a monoamine oxidase inhibitor (MAOI) used for psychiatric disorders, or within 14 days of stopping one. Do not start such an MAOI within 14 days of stopping Wellbutrin SR. Do not start Wellbutrin SR while receiving linezolid or intravenous methylene blue. These combinations can cause dangerous increases in blood pressure. Wellbutrin SR can cause seizures. Stop taking it if a seizure occurs. It can also raise blood pressure, which should be checked before treatment and periodically during treatment. Screening for bipolar disorder and monitoring for mania or hypomania are needed. People with untreated narrow drainage angles in the eyes are at risk of angle-closure glaucoma. Contact a healthcare professional if psychosis or other serious changes in thinking, mood or behaviour occur. Serious mental health reactions, including suicidal thoughts and behaviour, have been reported in people taking bupropion while trying to quit smoking. If these occur in this setting, stop taking bupropion and contact a healthcare professional.

Interactions

Tell your doctor if you take any of the following: • Ritonavir, lopinavir, efavirenz, carbamazepine, phenobarbital or phenytoin. These may affect treatment with Wellbutrin SR. • Antidepressants such as venlafaxine, nortriptyline, imipramine, desipramine, paroxetine, fluoxetine or sertraline; antipsychotics such as haloperidol, risperidone or thioridazine; metoprolol; or heart rhythm medicines such as propafenone or flecainide. Wellbutrin SR can increase their levels in the blood. • Digoxin. Wellbutrin SR may lower digoxin levels, which should be monitored. • Medicines that increase the risk of seizures. These require caution when used with Wellbutrin SR. • Levodopa or amantadine. Taking these with Wellbutrin SR can cause toxic effects on the brain and nervous system. • Monoamine oxidase inhibitors (MAOIs). Taking these with Wellbutrin SR increases the risk of high blood pressure reactions. Wellbutrin SR can also cause a false-positive urine test result for amphetamines. Tell the healthcare professional arranging the test that you take this medicine.

Dosing is set by your prescriber. Each health care professional may prescribe medication differently according to a patient's medical history and condition, so follow the directions on your prescription label and speak to your doctor or pharmacist before changing anything.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
