About Simponi

Overview

Simponi (golimumab) is a medicine that blocks tumor necrosis factor (TNF), a protein involved in inflammation. It treats adults with moderately to severely active rheumatoid arthritis in combination with methotrexate, active psoriatic arthritis with or without methotrexate, and active ankylosing spondylitis. It also treats moderately to severely active ulcerative colitis in adults and children weighing at least 15 kg. It works by binding to TNF and blocking its activity, although exactly how it treats ulcerative colitis is unknown.

What it treats

- Moderately to severely active rheumatoid arthritis in adults, in combination with methotrexate. - Active psoriatic arthritis in adults, alone or in combination with methotrexate. - Active ankylosing spondylitis in adults. - Moderately to severely active ulcerative colitis in adults and children weighing at least 15 kg.

Safety information

Side effects

Common side effects in adults include upper respiratory tract infections, inflammation of the nose and throat, and reactions at the injection site. Each occurred in more than 5% of adults. Contact a doctor promptly about serious side effects, including serious infections, new or worsening heart failure, new or worsening disorders that damage the protective covering of nerves, reactivation of hepatitis B, lupus-like syndrome, and low blood cell counts. Serious allergic reactions, including anaphylaxis, may occur and require immediate medical attention. Lymphoma occurred more often than in the general U.S. population. Other cancers have also been observed in people receiving TNF blockers. Do not start Simponi during an active infection. If an infection develops, contact your doctor; Simponi must be stopped if the infection becomes serious. People who carry hepatitis B need monitoring during treatment and for several months afterward.

When to call your doctor

- Seek immediate medical attention for a serious infection or an infection that is getting worse. - Get immediate medical help for a severe allergic reaction, including anaphylaxis. - Contact your doctor immediately for new or worsening symptoms of heart failure. - Contact your doctor promptly for new or worsening symptoms of a disorder affecting the protective covering of nerves. - Contact your doctor promptly if symptoms of a lupus-like illness develop.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

The label lists no formal contraindications for Simponi (golimumab). However, treatment should not start during an active infection. If an infection develops, careful monitoring is needed; Simponi should be stopped if the infection becomes serious. Caution is needed in people with heart failure, demyelinating disorders or hepatitis B infection. New or worsening heart failure or demyelinating disorders may occur. Stop Simponi if heart failure symptoms develop or worsen. Hepatitis B carriers need monitoring during treatment and for several months afterward; if the virus reactivates, Simponi should be stopped and antiviral treatment started. Lymphoma occurred more often than in the general U.S. population, and other cancers have been reported in people receiving TNF blockers. Invasive fungal infection should be considered if a widespread illness develops, particularly in people who live in or travel to areas where these infections are common. Simponi should be discontinued if a lupus-like syndrome develops. Serious allergic reactions, including anaphylaxis, may occur.

Interactions

Tell your doctor if you take abatacept or anakinra, as using either with Simponi increases the risk of serious infection. Avoid live vaccines and treatments that contain infectious agents while using Simponi.

Dosing

Storage

Store in the original carton to protect it from light. Check the pack for whether it must be refrigerated before first use and how long it may be kept at room temperature afterwards. Keep all medicines out of the sight and reach of children.
