Overview
Nifedipine extended-release tablets are a calcium channel blocker. They are used to treat high blood pressure and angina caused by spasms in the arteries supplying the heart. They are also used for chronic stable angina when beta blockers or nitrates do not adequately control symptoms or cannot be tolerated. Nifedipine reduces calcium entry into blood vessel muscle cells, helping arteries relax and widen to lower blood pressure and reduce the heart’s workload.
What it treats
- High blood pressure, alone or with other blood pressure medicines. - Vasospastic angina (chest pain caused by coronary artery spasms), including cases where spasm is suspected. - Chronic stable angina (chest pain brought on by exertion) without evidence of coronary artery spasm, when beta blockers and/or nitrates do not adequately control symptoms or cannot be tolerated.
Safety information
Side effects
Swelling from fluid retention (edema) was the most commonly reported side effect of nifedipine extended-release tablets. Swelling and headache were more common than with placebo. Less commonly reported effects include flushing, weakness, a pounding or racing heartbeat, trouble sleeping, nervousness, tingling, sleepiness, itching, rash, stomach pain, diarrhea, dry mouth, indigestion, gas, joint pain and leg cramps. Most of these occurred at rates similar to placebo. Contact a doctor if you experience fainting, low blood pressure, an irregular heartbeat, worsening angina (chest pain), or difficulty breathing. Bowel blockage and tablets sticking to the digestive tract wall and causing ulcers have been reported; some cases required hospitalization or surgery. Blockage has occurred even in people without previous digestive tract disease. Rare reports after marketing include serious skin reactions involving peeling or blistering, including Stevens–Johnson syndrome and toxic epidermal necrolysis. A widespread rash with pus-filled bumps has also been reported. These skin reactions warrant urgent medical attention.

Before taking this medicine
Do not take nifedipine if you have a known allergy to nifedipine.
Interactions
Avoid grapefruit and grapefruit juice while taking nifedipine, as they can increase the amount of nifedipine in your blood. Tell your doctor if you take: • Beta blockers: the combination may increase the risk of heart failure, very low blood pressure or worsening angina. • Digoxin: nifedipine may increase digoxin levels. Blood levels should be monitored when nifedipine is started, adjusted or stopped. • Coumarin anticoagulants: rare reports suggest that blood may take longer to clot, although the link to nifedipine is uncertain. • Cimetidine: this can increase nifedipine levels in the blood. • Phenytoin or other medicines that increase the activity of the enzyme CYP3A4: these can lower nifedipine levels. The label advises avoiding these combinations or considering another blood pressure medicine. • Fluconazole, itraconazole, clarithromycin, erythromycin, nefazodone, fluoxetine, saquinavir, indinavir or nelfinavir: these may increase nifedipine levels, so careful monitoring may be needed.
