FDA boxed warning
Because midodrine hydrochloride can cause marked elevation of supine blood pressure, it should be used in patients whose lives are considerably impaired despite standard clinical care. The indication for use of midodrine hydrochloride in the treatment of symptomatic orthostatic hypotension is based primarily on a change in a surrogate marker of effectiveness, an increase in systolic blood pressure measured one minute after standing, a surrogate marker considered likely to correspond to a clinical benefit. At present, however, clinical benefits of midodrine hydrochloride, principally improved ability to carry out activities of daily living, have not been verified.
About Proamatine
Overview
Proamatine is a prescription tablet containing midodrine hydrochloride, used to treat low blood pressure on standing that causes symptoms (symptomatic orthostatic hypotension). It works by raising standing blood pressure, although improved ability to carry out daily activities has not been established. It is reserved for people whose lives remain considerably affected despite standard care and lifestyle changes. It can cause a marked rise in blood pressure when lying down.
What it treats
Symptomatic orthostatic hypotension (low blood pressure on standing) that considerably impairs daily life despite standard care, including support stockings, measures to increase fluid volume and lifestyle changes.
Safety information
Side effects
Common side effects include high blood pressure when lying down or sitting, tingling and itching (mainly of the scalp), goosebumps, chills, an urgent or frequent need to urinate, and difficulty emptying the bladder. Less frequent effects include headache, pressure or fullness in the head, facial flushing, confusion, dry mouth, anxiety and rash. High blood pressure when lying down is the most potentially serious side effect identified in the label. Contact your doctor if this occurs. Urinary retention, being unable to empty your bladder, also warrants contacting a doctor.

Before taking this medicine
Do not take Proamatine (midodrine) if you have severe underlying heart disease, acute kidney disease, urinary retention (inability to empty your bladder), pheochromocytoma (a type of adrenal gland tumour), or thyrotoxicosis (excess thyroid hormone). It should also not be used if your blood pressure remains excessively high when lying down.
Interactions
Cardiac glycosides taken with midodrine may cause or worsen a slow heartbeat, impaired electrical conduction in the heart (AV block), or an irregular heartbeat. Avoid taking midodrine with medicines that raise blood pressure, including phenylephrine, pseudoephedrine, ephedrine, dihydroergotamine, thyroid hormones or droxidopa. If combined use cannot be avoided, blood pressure needs close monitoring. MAO inhibitors and linezolid should also be avoided. Tell your doctor if you take fludrocortisone or use salt supplements. These combinations need careful monitoring for high blood pressure while lying down. Your doctor may need to adjust fludrocortisone treatment or your salt intake. Alpha-blockers such as prazosin, terazosin and doxazosin may reduce midodrine’s effects.

Dosing

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
