About Plaquenil
Overview
Plaquenil tablets contain hydroxychloroquine sulfate, an antimalarial and antirheumatic medicine. It treats rheumatoid arthritis, systemic lupus erythematosus and chronic discoid lupus erythematosus in adults. It also treats certain types of uncomplicated malaria in adults and children and helps prevent malaria in areas where chloroquine resistance has not been reported; it is not recommended for complicated malaria or resistant strains. How it reduces inflammation and affects the immune system in rheumatoid arthritis and lupus is not fully understood.
What it treats
- Uncomplicated malaria caused by Plasmodium falciparum, P. malariae, P. ovale or P. vivax in adults and children, where chloroquine resistance is not present. - Malaria prevention in adults and children travelling to or living in areas where chloroquine resistance is not reported. - Rheumatoid arthritis in adults. - Systemic lupus erythematosus in adults. - Chronic discoid lupus erythematosus in adults.
Safety information
Side effects
The most commonly reported side effects of Plaquenil are nausea, vomiting, diarrhea and abdominal pain. Other reported effects include headache, dizziness, fatigue, loss of appetite, weight loss, itching, rash, hair loss and changes in hair color. Contact a doctor promptly for vision changes, hearing loss, muscle weakness, seizures, mood or behavior changes, or signs of low blood sugar. Serious reported effects include heart muscle damage, heart failure and abnormal heart rhythms, which can be life-threatening or fatal; permanent retinal damage; blood cell disorders; liver failure; and kidney toxicity. Eye examinations are recommended before and during treatment. Serious allergic and skin reactions have also been reported, including swelling, breathing difficulties, and severe skin reactions such as Stevens–Johnson syndrome and toxic epidermal necrolysis. Seek urgent medical help for these reactions or for suicidal thoughts or behavior. Other serious mental health effects include depression, hallucinations, confusion and psychosis. Plaquenil can worsen psoriasis and carries risks in people with porphyria. The label advises avoiding it in these conditions. For many reactions reported after approval, the frequency and whether the medicine caused them cannot be reliably established.
When to call your doctor
If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take Plaquenil if you have a known allergy to hydroxychloroquine or other 4-aminoquinoline compounds. It should be avoided in people with psoriasis or porphyria. Liver toxicity has been reported in people with porphyria cutanea tarda. Important precautions include potentially life-threatening heart muscle damage and abnormal heart rhythms, irreversible retinal damage, serious skin reactions, reduced blood cell production and kidney toxicity. The risk of retinal damage is related to the total amount taken over time and the duration of treatment. A retinal examination is recommended before treatment and further examinations during treatment. Plaquenil should be discontinued if bone marrow suppression occurs or kidney toxicity is suspected. In people with underlying connective tissue disorders, phospholipidosis, a buildup of certain fats in cells, should be considered as a possible cause of kidney injury.
Interactions
Tell your doctor if you take medicines that prolong the QT interval (a change in the heart’s electrical activity) or other medicines that can cause abnormal heart rhythms. These are important potential interactions with Plaquenil.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
