About Monopril

Overview

These tablets contain a combination of fosinopril sodium and hydrochlorothiazide. They treat high blood pressure but are not intended as the first treatment. Fosinopril is converted in the body to an active substance that blocks an enzyme called ACE, while hydrochlorothiazide acts as a diuretic.

What it treats

High blood pressure (hypertension). This fixed-dose combination is not intended as initial treatment.

Safety information

Side effects

Commonly reported side effects include headache, cough, tiredness, dizziness, upper respiratory infections, and muscle or joint pain. Other reported effects include nausea, vomiting, diarrhoea, heartburn, abdominal pain, itching, rash, muscle cramps, sleepiness, and tingling or numbness. Seek urgent medical help for angioedema, a swelling reaction that can affect the head, neck or intestines. Contact a doctor promptly if you experience chest pain, fainting, an irregular heartbeat, or dizziness when standing, which may reflect a fall in blood pressure. Blood tests may show changes in electrolytes, including magnesium and calcium, as well as uric acid, blood glucose, cholesterol and triglycerides. Neutropenia, a low level of a type of infection-fighting white blood cell, has also been reported.

Before taking this medicine

Do not take this medicine if you cannot produce urine (anuria), or if you are allergic to fosinopril, any other ACE inhibitor, hydrochlorothiazide, other sulfonamide-derived medicines, or any ingredient in the tablets. Tell your prescriber if you have a history of allergies or bronchial asthma, as allergic reactions are more likely in these patients.

Interactions

Tell your doctor if you take any of the following with fosinopril and hydrochlorothiazide: Potassium supplements or potassium-sparing diuretics, such as spironolactone, amiloride or triamterene, can increase the risk of high blood potassium. Frequent potassium blood tests may be needed. Lithium levels can rise, increasing the risk of lithium toxicity. Frequent lithium blood tests are recommended if these medicines are used together. Other medicines acting on the renin-angiotensin system, including angiotensin receptor blockers, other ACE inhibitors or aliskiren, increase the risk of low blood pressure, high potassium and kidney problems, including acute kidney failure. Close monitoring is needed. Aliskiren must not be used with this combination in people with diabetes and should be avoided in people with impaired kidney function. Antacids may reduce fosinopril absorption. Cholestyramine and colestipol can reduce hydrochlorothiazide absorption. Nonsteroidal anti-inflammatory drugs (NSAIDs) may reduce the fluid-removing and blood-pressure-lowering effects of thiazide diuretics. Their effect on blood pressure control with this specific combination has not been studied. Insulin requirements may change. Hydrochlorothiazide may also reduce the effectiveness of methenamine. Injectable gold, such as sodium aurothiomalate, has rarely caused facial flushing, nausea, vomiting and low blood pressure when used with ACE inhibitors. Thiazides may increase the effects of tubocurarine, a muscle relaxant, and reduce responsiveness to norepinephrine. Tell your treating team if these medicines are being considered.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
