About Micardis

Overview

Micardis contains telmisartan, an angiotensin II receptor blocker (ARB). It is used to treat high blood pressure and to reduce cardiovascular risk in patients who cannot take ACE inhibitors. It works by blocking the effects of angiotensin II, a substance that narrows blood vessels and stimulates the release of aldosterone, a hormone involved in retaining salt.

Side effects

Common side effects reported in high blood pressure trials include back pain, sinusitis (sinus inflammation) and diarrhea. Serious problems that warrant contacting a doctor include low blood pressure and kidney problems, including kidney dysfunction when used with ramipril. Serious adverse events reported in cardiovascular risk reduction trials included intermittent claudication (leg pain during walking) and skin ulcers (open sores). Exposure during pregnancy can harm the fetus or newborn. Contact your doctor immediately if you become pregnant while taking Micardis.

When to call your doctor

- Contact your doctor immediately if you become pregnant. Micardis must be stopped as soon as pregnancy is detected because it can seriously harm or cause the death of an unborn baby.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Micardis if you have a known allergy to telmisartan or any other ingredient, including a history of anaphylaxis or angioedema (serious swelling) caused by them. Avoid use during pregnancy because the unborn baby should not be exposed to this medicine. If breastfeeding, a decision is needed about whether to stop breastfeeding or stop Micardis. Micardis requires caution if you have fluid or salt depletion, as low blood pressure can occur. These problems should be corrected before treatment, and signs of low blood pressure should be monitored. Liver or kidney problems require careful monitoring. Some older adults may be more sensitive to its effects. Avoid taking Micardis together with an ACE inhibitor.

Interactions

Micardis (telmisartan) can interact with: - Digoxin: Telmisartan can increase digoxin levels in the blood. Levels should be monitored when telmisartan is started, adjusted or stopped. - Lithium: Taking these medicines together can increase lithium levels and cause toxicity. Blood lithium levels should be monitored during combined use. - Non-steroidal anti-inflammatory drugs (NSAIDs), including COX-2 inhibitors: These can reduce the blood-pressure-lowering effect of telmisartan and increase the risk of kidney problems, including acute kidney failure. The risk is greater in older people, those with reduced body fluid levels, including people taking diuretics, and those with existing kidney problems. Kidney function should be monitored periodically during combined use. - Ramipril: Telmisartan increases exposure to ramipril and its active metabolite, which may increase their combined effects. Taking Micardis with ramipril is not recommended. Raise these medicines with your doctor if you take them. Telmisartan may also slow the breakdown of medicines processed by the liver enzyme CYP2C19.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
