Lymphoma and other malignancies, some fatal, have been reported in children and adolescent patients treated with TNF blockers including adalimumab products.

Post-marketing cases of hepatosplenic T-cell lymphoma (HSTCL), a rare type of T-cell lymphoma, have occurred in adolescent and young adults with inflammatory bowel disease treated with TNF blockers including adalimumab products.

About Idacio

Overview

Idacio contains adalimumab-aacf, a monoclonal antibody known as a tumor necrosis factor (TNF) blocker. It is used in adults for certain forms of rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, Crohn’s disease, ulcerative colitis, plaque psoriasis, hidradenitis suppurativa and non-infectious uveitis. It is also used for moderately to severely active polyarticular juvenile idiopathic arthritis in children aged 2 years and older, and moderately to severely active Crohn’s disease in children aged 6 years and older. It works by blocking TNF-alpha, a protein involved in inflammation and immune responses.

What it treats

• Moderately to severely active rheumatoid arthritis in adults. • Moderately to severely active polyarticular juvenile idiopathic arthritis in patients aged 2 years and older. • Active psoriatic arthritis in adults. • Active ankylosing spondylitis in adults. • Moderately to severely active Crohn’s disease in patients aged 6 years and older. • Moderately to severely active ulcerative colitis in adults. Effectiveness has not been established in patients who have lost response to or could not tolerate TNF blockers. • Moderate to severe chronic plaque psoriasis in adults who are candidates for systemic therapy or phototherapy, when other systemic therapies are medically less appropriate. • Moderate to severe hidradenitis suppurativa in adults. • Non-infectious intermediate, posterior and panuveitis in adults.

Safety information

Side effects

Common side effects include infections such as upper respiratory infections and sinusitis, injection site reactions, headache and rash. Serious risks include severe infections, including invasive fungal infections; severe allergic reactions, including anaphylaxis; reactivation of hepatitis B; new or worsening nerve disorders involving damage to the protective covering of nerves; low blood cell counts; new or worsening heart failure; and a lupus-like syndrome. Contact a doctor promptly if a serious reaction is suspected. Seek immediate medical attention for a severe allergic reaction or symptoms of low blood cell counts. Cancers occurred more often in patients treated with adalimumab than in those in the comparison groups. Hepatitis B carriers need monitoring during treatment and for several months afterwards.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

The label lists no formal contraindications for Idacio. However, treatment should not start during an active infection. If an infection develops during treatment, it requires careful monitoring; Idacio should be stopped if the infection becomes serious. Conditions requiring caution include hepatitis B infection, demyelinating disease, low blood cell counts and heart failure. Hepatitis B carriers need monitoring during treatment and for several months afterwards. If hepatitis B reactivates, Idacio should be stopped and antiviral treatment started. New or worsening demyelinating disease or heart failure may occur. Symptoms suggesting low blood cell counts require immediate medical attention, and stopping treatment may be considered. Serious allergic reactions, including anaphylaxis, may occur. Cancer occurred more often in patients treated with adalimumab than in those in the comparison groups. A lupus-like syndrome requires stopping Idacio. Patients who become generally unwell and live in or have travelled to areas where certain fungal infections are common may need treatment for a suspected invasive fungal infection.

Interactions

Tell your doctor if you use abatacept or anakinra, as combining either with Idacio increases the risk of serious infection. Avoid live vaccines while using Idacio.

Store below 25°C in a dry place with the bottle tightly closed, away from direct light and moisture. Keep all medicines out of the sight and reach of children.
