About Diovan HCT

Overview

Diovan HCT is a combination tablet containing valsartan and hydrochlorothiazide. It treats high blood pressure and may be used when one medicine alone is not enough or when more than one medicine is likely to be needed. Valsartan blocks a substance that narrows blood vessels, while hydrochlorothiazide helps the kidneys remove more salt and water through urine.

Side effects

Reported side effects include inflammation of the nose and throat (nasopharyngitis), headache and dizziness. Headache and dizziness were the most common reasons people stopped treatment. Contact a doctor if you develop signs of low blood pressure or a fluid or electrolyte imbalance. Other serious concerns include kidney function problems, changes in potassium levels, new or worsening systemic lupus erythematosus, and acute angle-closure glaucoma, a serious eye condition. Kidney function and potassium levels may need monitoring in susceptible patients.

When to call your doctor

- If you find out you are pregnant, stop taking Diovan HCT and contact your doctor immediately. It can cause injury or death to an unborn baby.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Diovan HCT if you cannot produce urine (anuria), are allergic to any of its ingredients or to other sulfonamide-derived medicines, or have diabetes and take aliskiren. Conditions requiring caution include low blood pressure, fluid depletion, fluid or electrolyte imbalances, and systemic lupus erythematosus, which may develop or worsen. Fluid depletion should be corrected before treatment. Kidney function and potassium levels should be monitored in susceptible patients. Acute angle-closure glaucoma is also a warning associated with this medicine. Breastfeeding is not recommended during treatment.

Interactions

Lithium can build up in the blood and cause toxicity when taken with Diovan HCT. Blood lithium levels need monitoring during combined use. Potassium supplements, potassium-containing salt substitutes, potassium-sparing diuretics such as spironolactone, triamterene or amiloride, and other medicines that raise potassium, such as heparin, can increase blood potassium levels. Potassium monitoring may be needed. Other medicines that act on the renin-angiotensin system, including ACE inhibitors, other angiotensin receptor blockers and aliskiren, can increase the risk of low blood pressure, high potassium and kidney problems, including acute kidney failure. These combinations should generally be avoided. Aliskiren must not be combined with Diovan HCT in people with diabetes and should be avoided in people with reduced kidney function. NSAIDs, including COX-2 inhibitors, can reduce the blood-pressure-lowering and diuretic effects of Diovan HCT and increase the risk of kidney problems. This risk is greater in older adults, people who are dehydrated and those with impaired kidney function. Kidney function and treatment effects may need monitoring. Diabetes medicines, including insulin, may need adjustment by the prescriber. Carbamazepine may cause low blood sodium with symptoms when used with Diovan HCT. Cyclosporine may increase uric acid levels and the risk of gout-related problems. Cholestyramine and colestipol can reduce absorption of hydrochlorothiazide. Ask your doctor how to manage this interaction.
