Overview
Diltiazem is a calcium channel blocker used to treat high blood pressure. It is also used to improve exercise tolerance in people with chronic stable angina. It works by reducing calcium entry into heart and blood vessel muscle cells, helping blood vessels relax and reducing the heart’s need for oxygen.

Safety information
Side effects
Common side effects include swelling of the lower legs, sinus congestion and rash in people treated for high blood pressure. In people treated for angina, common side effects include swelling of the lower legs, headache, dizziness, tiredness, a slow heartbeat, first-degree heart block (a delay in the heart’s electrical signals) and cough. Contact a doctor promptly about a slow heartbeat or heart block, heart failure, increased liver enzymes or sudden liver injury, or severe skin reactions. Heart rate and rhythm, and signs and symptoms of heart failure, should be monitored.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take diltiazem if you have: - Sick sinus syndrome, unless you have a functioning ventricular pacemaker. - Second- or third-degree heart block, unless you have a functioning ventricular pacemaker. - Very low blood pressure, with a systolic reading below 90 mm Hg. - An allergy to diltiazem. Caution and monitoring are needed for a slow heartbeat, heart block or heart failure. The label also warns of increased liver enzymes, acute liver injury and severe skin reactions. The supplied label lists another contraindication involving an acute heart attack and a pulmonary condition, but the wording is incomplete. Ask your prescriber to clarify this restriction.
Interactions
Tell your doctor if you take beta-blockers, digitalis, or other medicines that affect the heart’s pumping action or electrical signals. Taking these with diltiazem may increase the risk of low blood pressure, a slow heartbeat, or heart failure. Diltiazem is processed by and also blocks a liver enzyme called CYP3A4. Medicines processed by this enzyme may need dose adjustments by your prescriber.

Storage
