About Zepzelca
Overview
Zepzelca (lurbinectedin) is an alkylating cancer medicine given by intravenous infusion. It treats adults with metastatic small cell lung cancer that has progressed during or after platinum-based chemotherapy; this use has accelerated FDA approval, and continued approval may depend on confirmation of clinical benefit. It is also used with atezolizumab or atezolizumab and hyaluronidase-tqjs as maintenance treatment for adults with extensive-stage small cell lung cancer that has not progressed after initial treatment with one of these combinations plus carboplatin and etoposide. It works by binding to DNA and disrupting processes involved in cell activity and repair, leading to cell death.
What it treats
• Maintenance treatment of adults with extensive-stage small cell lung cancer that has not progressed after initial treatment with atezolizumab (with or without hyaluronidase-tqjs), carboplatin and etoposide. Zepzelca is used with atezolizumab or atezolizumab and hyaluronidase-tqjs. • Treatment of adults with metastatic small cell lung cancer that has progressed during or after platinum-based chemotherapy. This use has accelerated approval based on tumour response rate and how long responses lasted; continued approval may depend on confirmation of clinical benefit in further trials.
Safety information
Side effects
Common side effects of Zepzelca used alone include tiredness, nausea, vomiting, reduced appetite, constipation, diarrhea, muscle and joint pain, shortness of breath and cough. Blood tests may show low white blood cells, red blood cells or platelets; increased creatinine, liver enzymes or blood sugar; and low albumin, sodium or magnesium. When used with atezolizumab, the most common effects include low blood cell counts, nausea, tiredness and weakness. Serious side effects that warrant prompt medical attention include suppression of blood cell production, liver damage, severe muscle breakdown (rhabdomyolysis), and leakage of the infusion into surrounding tissue, which can cause tissue death. Tell your healthcare team immediately if you notice pain, swelling or other changes around the infusion site. Blood counts, liver function and muscle enzyme levels are monitored during treatment. Zepzelca can harm an unborn baby. Contact your doctor if you become pregnant or think you may be pregnant during treatment.

Before taking this medicine
The FDA label lists no formal contraindications for Zepzelca. However, use should be avoided in people with severe liver impairment. Moderate liver impairment requires a prescriber-directed dose reduction. Zepzelca can suppress bone marrow function and lower blood cell counts. Blood counts must be checked before each administration, and treatment should begin only when neutrophil and platelet counts meet the label’s minimum requirements. Liver function and creatine phosphokinase (CPK) levels also need monitoring because of the risks of liver injury and serious muscle breakdown (rhabdomyolysis). These problems may require treatment to be paused, reduced or stopped. Leakage outside the vein during infusion can cause tissue death. The infusion site must be monitored, and the infusion stopped immediately if leakage occurs. Zepzelca can harm an unborn baby. Females and males of reproductive potential should use effective contraception. Do not breastfeed during treatment.
Interactions
Zepzelca should not be used with strong or moderate CYP3A inhibitors or strong CYP3A inducers. Ask your doctor to check whether any of your other medicines fall into these groups before combining them with Zepzelca.
