About Zemplar
Overview
Zemplar is a prescription medicine containing paricalcitol, supplied as capsules. It is used in adults to prevent and treat secondary hyperparathyroidism associated with chronic kidney disease stages 3 and 4, or stage 5 in people receiving hemodialysis or peritoneal dialysis. Paricalcitol is a synthetic vitamin D analog, meaning it acts like vitamin D in the body.
What it treats
- Prevention and treatment of secondary hyperparathyroidism in adults with chronic kidney disease (CKD) stages 3 or 4. - Prevention and treatment of secondary hyperparathyroidism in adults with CKD stage 5 who are receiving hemodialysis or peritoneal dialysis.
Safety information
Side effects
Common side effects include diarrhea, inflammation of the nose and throat, dizziness, vomiting, high blood pressure, allergic reactions, nausea, and swelling from fluid buildup. Contact your doctor if you develop an allergic reaction. Excessive use can cause high calcium levels in the blood or urine, high blood phosphorus, excessive suppression of parathyroid hormone (PTH), and a bone disorder called adynamic bone disease. High blood calcium can also increase the risk of digitalis toxicity if you take a digitalis medicine. Blood tests are needed to monitor calcium, phosphorus, and PTH. Paricalcitol may also increase blood creatinine, lowering the estimated kidney filtration rate (eGFR).
Before taking this medicine
Do not take Zemplar (paricalcitol) if you have high blood calcium levels or vitamin D toxicity. Breastfeeding is not recommended during treatment. Use caution if you take digitalis medicines, as high blood calcium can increase their toxicity. Prescription vitamin D medicines and derivatives should be withheld during treatment. Avoid excessive use of aluminum-containing products. Excessive paricalcitol use can suppress parathyroid hormone (PTH) too much, raise calcium in the blood and urine, raise blood phosphorus, and cause bone disease with reduced bone turnover. Blood calcium, phosphorus and PTH levels need monitoring when treatment starts and after dose changes. Paricalcitol may also raise blood creatinine, lowering the estimated measure of kidney filtration (eGFR).
Interactions
Tell your doctor if you take medicines such as ketoconazole, itraconazole, clarithromycin or ritonavir, or drink grapefruit juice. These can increase the amount of paricalcitol in your body. Your doctor may need to adjust your treatment and closely monitor parathyroid hormone (iPTH) and blood calcium levels when these medicines are started or stopped. Cholestyramine and mineral oil can reduce paricalcitol absorption when taken at the same time. Other substances that affect fat absorption may also interfere. Ask your doctor how to separate these from your paricalcitol capsules.

Storage
