About Zejula
Overview
Zejula (niraparib) is a prescription medicine used as maintenance treatment for certain epithelial ovarian, fallopian tube or primary peritoneal cancers in adults. It is used for advanced cancer that tests positive for homologous recombination deficiency (HRD) after a complete or partial response to first-line platinum-based chemotherapy. It is also used when these cancers return and have a harmful or suspected harmful inherited BRCA mutation, following a complete or partial response to platinum-based chemotherapy. It blocks PARP enzymes involved in repairing DNA, which can lead to DNA damage and cancer cell death.
What it treats
• Maintenance treatment for adults with advanced epithelial ovarian, fallopian tube or primary peritoneal cancer that has responded fully or partly to first-line platinum-based chemotherapy and is homologous recombination deficiency (HRD)-positive due to a harmful or suspected harmful BRCA mutation and/or genomic instability. • Maintenance treatment for adults with recurrent epithelial ovarian, fallopian tube or primary peritoneal cancer that has responded fully or partly to platinum-based chemotherapy and has an inherited harmful or suspected harmful BRCA mutation. An FDA-authorized companion diagnostic test is used to select patients for treatment.

Side effects
Common side effects of Zejula include nausea, vomiting, constipation, diarrhea, abdominal pain, decreased appetite, tiredness, muscle and joint pain, headache, dizziness, trouble sleeping, shortness of breath, cough, rash, high blood pressure, urinary tract infection, sudden kidney injury and low magnesium levels. Low platelet, red blood cell and white blood cell counts are also common. Serious side effects that warrant contacting a doctor include bone marrow suppression, high blood pressure and cardiovascular effects, and myelodysplastic syndrome or acute myeloid leukemia (MDS/AML). Some cases of MDS/AML have been fatal. Posterior reversible encephalopathy syndrome (PRES), a serious condition affecting the brain, has also occurred. Regular blood counts and blood pressure and heart rate checks are needed during treatment. Zejula can harm an unborn baby. Use effective contraception if you can become pregnant, and contact your doctor if you become pregnant during treatment.
Before taking this medicine
The FDA label lists no contraindications for Zejula. However, important precautions apply: Zejula can cause bone marrow suppression and has been associated with myelodysplastic syndrome and acute myeloid leukemia (MDS/AML), including fatal cases. Complete blood counts should be checked weekly for the first month, monthly for the next 11 months and periodically thereafter. Treatment must be stopped if MDS/AML is confirmed. Blood pressure and heart rate require monitoring because of the risk of high blood pressure and cardiovascular effects: at least weekly for the first two months, monthly for the first year and periodically thereafter. Posterior reversible encephalopathy syndrome (PRES) has occurred during treatment. Zejula must be stopped if PRES is confirmed. Zejula can harm an unborn baby. People who can become pregnant should use effective contraception. Do not breastfeed during treatment.

Storage
