About Xpovio
Overview
Xpovio contains selinexor, a nuclear export inhibitor used to treat adults with multiple myeloma. It is used with bortezomib and dexamethasone after at least one prior treatment. It is also used with dexamethasone when multiple myeloma has returned or resisted treatment after at least four prior therapies, and is resistant to at least two proteasome inhibitors, two immunomodulatory agents and an anti-CD38 monoclonal antibody. In laboratory studies, selinexor blocked a protein called XPO1, keeping tumour-suppressing proteins inside the cell nucleus and helping stop cancer cell growth.
What it treats
- Multiple myeloma in adults who have received at least one previous treatment, used with bortezomib and dexamethasone. - Multiple myeloma that has returned or has not responded to treatment in adults who have received at least four previous treatments and whose disease is resistant to at least two proteasome inhibitors, at least two immunomodulatory agents and an anti-CD38 monoclonal antibody, used with dexamethasone.
Safety information
Side effects
Common side effects of Xpovio include tiredness, nausea, vomiting, reduced appetite, weight loss, diarrhea, constipation, shortness of breath, upper respiratory tract infections, problems with nerves outside the brain and spinal cord, and cataracts. Low blood cell counts and low sodium levels are also common. Side effects vary depending on the treatment combination used. Serious side effects include low platelet counts, low white blood cell counts, anemia, low sodium or phosphate levels, serious infections, and neurological problems such as dizziness or changes in mental state. Contact your doctor promptly if you develop an infection, dizziness, changes in mental state, or severe nausea, vomiting or diarrhea. Do not drive or perform hazardous activities while neurological symptoms continue. Cataracts may develop or worsen and usually require surgery. Blood tests are needed throughout treatment to monitor platelet counts, certain white blood cells and sodium levels. Your prescriber may need to interrupt, reduce or stop treatment to manage side effects.

Before taking this medicine
The FDA label lists no contraindications for Xpovio (selinexor). However, treatment requires monitoring and caution because it can cause low platelet counts, low neutrophil counts, low blood sodium, serious infections, nausea, vomiting, diarrhea, loss of appetite and weight loss. Platelet, neutrophil and sodium levels should be monitored throughout treatment, and infections should be treated promptly. Xpovio may cause dizziness or changes in mental state. Do not drive or take part in hazardous activities until these effects resolve. Cataracts may develop or worsen and usually require surgery. Xpovio can harm an unborn baby. Females who could become pregnant and males with a female partner who could become pregnant should use effective contraception. Do not breastfeed during treatment.
Interactions
Tell your doctor or pharmacist about all medicines, vitamins, herbal products and supplements you take before starting Xpovio. Medicines that cause dizziness, drowsiness or confusion may worsen these effects of selinexor. These include sleeping medicines, sedatives, opioid pain medicines and some antihistamines. Your doctor may need to review their use together. Discuss alcohol use with your doctor, as it may also worsen dizziness or drowsiness. Check before starting any new medicine or supplement during treatment.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
