About Xalkori

Overview

Xalkori (crizotinib) is a prescription kinase inhibitor used to treat adults with non-small cell lung cancer that has spread to other parts of the body and is ALK- or ROS1-positive, as confirmed by an FDA-approved test. It also treats ALK-positive systemic anaplastic large cell lymphoma that has returned or has not responded to treatment in children aged 1 year and older and young adults; safety and effectiveness for this use have not been established in older adults. It is also used in adults and children aged 1 year and older with ALK-positive inflammatory myofibroblastic tumors that cannot be removed surgically, have returned, or have not responded to treatment. It works by blocking certain proteins, including ALK and ROS1, involved in signals that help cancer cells grow and survive.

What it treats

- Metastatic non-small cell lung cancer (NSCLC) in adults whose tumors are ALK-positive or ROS1-positive, as detected by an FDA-approved test. - ALK-positive systemic anaplastic large cell lymphoma (ALCL) that has returned or has not responded to treatment, in children aged 1 year and older and young adults. Safety and effectiveness have not been established in older adults with this condition. - ALK-positive inflammatory myofibroblastic tumor (IMT) that cannot be removed by surgery, has returned, or has not responded to treatment, in adults and children aged 1 year and older.

Safety information

Side effects

Common side effects vary by age and the condition being treated. They include vision changes, nausea, vomiting, diarrhea, constipation, swelling, tiredness, reduced appetite, dizziness, headache, abdominal pain, muscle and joint pain, mouth inflammation or sores, rash, itching, fever, cough, upper respiratory infections, and nerve problems. Increased liver enzymes can also occur. Contact your doctor promptly about vision changes or severe nausea, vomiting, diarrhea, or mouth sores. Serious risks include liver damage, which can be fatal; lung inflammation; changes in the heart’s electrical rhythm (QT prolongation); a slow heartbeat; and severe vision loss. Severe digestive side effects are a particular concern in children and young adults with ALCL and children with IMT. Severely reduced levels of certain white blood cells and platelets have also been reported in patients with ALCL. Your doctor will monitor liver tests, heart rate, blood pressure, and eye health during treatment. Some patients also need heart rhythm and blood electrolyte checks. Serious side effects may require treatment to be paused, adjusted, or stopped by the prescriber. Xalkori can harm an unborn baby. Effective contraception is advised for females who could become pregnant.

When to call your doctor

• Seek immediate medical attention for severe loss of vision. • Seek immediate medical attention for a noticeably slow heartbeat. • Seek immediate medical attention for severe nausea, vomiting, diarrhea or mouth sores, particularly in children and young adults.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

The FDA label lists no contraindications for Xalkori. However, it carries serious warnings and requires monitoring during treatment. Xalkori can cause liver injury, including fatal liver injury, and requires periodic liver tests. It can also cause lung inflammation (interstitial lung disease or pneumonitis), which requires permanent discontinuation. Caution and heart monitoring are needed in people with a history of, or risk factors for, QT prolongation, or those taking medicines that prolong the QT interval. Xalkori can also slow the heart rate; heart rate and blood pressure should be checked regularly. Vision changes, including severe vision loss, can occur. Eye monitoring is needed throughout treatment, and Xalkori must be discontinued if severe vision loss occurs. Severe nausea, vomiting, diarrhoea and inflammation of the mouth can occur in children and young adults with anaplastic large cell lymphoma (ALCL), and in children with inflammatory myofibroblastic tumour (IMT). Xalkori can harm an unborn baby. Females who could become pregnant should use effective contraception. Do not breastfeed during treatment.

Interactions

Avoid taking Xalkori with medicines that strongly inhibit or induce CYP3A, an enzyme involved in processing medicines. Also avoid medicines processed by CYP3A when even small changes in their levels could cause serious side effects. Ask your doctor to check whether any of your other medicines fall into these groups.

Storage
