FDA boxed warningRisk of hepatitis b virus reactivation in patients coinfected with hcv and hbv

Test all patients for evidence of current or prior hepatitis B virus (HBV) infection before initiating treatment with VOSEVI. HBV reactivation has been reported in HCV/HBV coinfected patients who were undergoing or had completed treatment with HCV direct-acting antivirals (DAA) and were not receiving HBV antiviral therapy. Some cases have resulted in fulminant hepatitis, hepatic failure, and death. Monitor HCV/HBV coinfected patients for hepatitis flare or HBV reactivation during HCV treatment and post-treatment follow-up. Initiate appropriate patient management for HBV infection as clinically indicated.

About Vosevi

Overview

Vosevi is a tablet containing three antiviral medicines: sofosbuvir, velpatasvir and voxilaprevir. It treats chronic hepatitis C in adults without cirrhosis or with compensated cirrhosis who have previously received certain hepatitis C treatments. It is used for genotypes 1–6 after treatment containing an NS5A inhibitor, or genotypes 1a or 3 after treatment containing sofosbuvir without an NS5A inhibitor. The three ingredients block different proteins the hepatitis C virus needs to multiply.

What it treats

Vosevi is used to treat chronic hepatitis C in adults without cirrhosis or with compensated cirrhosis (Child-Pugh A) who have: - Genotype 1, 2, 3, 4, 5 or 6 infection previously treated with a regimen containing an NS5A inhibitor. - Genotype 1a or 3 infection previously treated with a regimen containing sofosbuvir without an NS5A inhibitor.

Safety information

Side effects

Common side effects of Vosevi include headache, tiredness, diarrhea and nausea. Serious risks that warrant contacting a doctor include: • Reactivation of hepatitis B in people with current or previous infection, which may cause a flare of hepatitis. Testing is needed before treatment, with monitoring during and after treatment for people with both hepatitis B and C. • Worsening liver function or liver failure, including fatal cases, reported mostly in people with cirrhosis and moderate or severe liver impairment. • A dangerously slow heartbeat when taken with amiodarone. The risk is particularly concerning in people also taking beta blockers or who have heart conditions or advanced liver disease. Taking amiodarone with Vosevi is not recommended; if no alternative treatment is available, heart monitoring is recommended.

When to call your doctor

- Seek medical attention immediately for a slow heartbeat, especially if you take amiodarone with Vosevi.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Vosevi with rifampin. Before treatment, all patients should be tested for current or past hepatitis B infection. Hepatitis B can reactivate during hepatitis C treatment. Patients with both infections need monitoring during treatment and follow-up, and hepatitis B management when needed. Advanced liver disease requires caution. Worsening liver function or liver failure, including fatal cases, has been reported with regimens containing an HCV NS3/4A protease inhibitor, mainly in patients with cirrhosis and moderate or severe liver impairment. Monitoring is needed, and Vosevi must be stopped if liver function deteriorates or liver failure develops. Taking Vosevi with amiodarone is not recommended because serious slowing of the heartbeat with symptoms may occur. The risk is particularly concerning in people also taking beta blockers or who have underlying heart conditions or advanced liver disease. If no suitable alternative treatment is available, heart monitoring is recommended.

Interactions

Tell your doctor about all medicines and supplements you take. St. John’s wort and carbamazepine may lower levels of Vosevi’s active ingredients, and taking them with Vosevi is not recommended. Other medicines that increase the activity of certain drug transporters or liver enzymes may have the same effect. Your doctor should check the full prescribing information for potential interactions. Clearing hepatitis C can change liver function and affect how other medicines work. More frequent monitoring of blood glucose or INR (a blood-clotting measure) may be needed, and your doctor may need to adjust other medicines.

Storage
