About VITRAKVI
Overview
VITRAKVI capsules contain larotrectinib, a kinase inhibitor used to treat certain solid tumors in adults and children. These tumors must have an NTRK gene fusion without a known acquired resistance mutation, identified through an FDA-approved test. Treatment is for tumors that have spread or would require surgery likely to cause serious harm, when there are no satisfactory alternatives or the cancer has progressed after treatment. Larotrectinib blocks TRK proteins that can help these cancer cells grow and survive.
What it treats
VITRAKVI is used to treat solid tumors in adults and children when all of the following apply: - The tumor has an NTRK gene fusion without a known acquired resistance mutation. - The cancer has spread to other parts of the body, or surgery is likely to cause serious harm. - There are no satisfactory alternative treatments, or the cancer has progressed after treatment. Patients are selected for treatment using an FDA-approved test.
Safety information
Side effects
Common side effects include muscle, bone or joint pain, tiredness, vomiting, cough, constipation, fever, diarrhea, nausea, abdominal pain, dizziness and rash. Common blood test changes include increased liver enzymes, anemia, low white blood cell counts, low albumin and low calcium. Contact your doctor if you develop dizziness, problems with thinking, mood changes or sleep disturbances. Do not drive or operate hazardous machinery if you experience these nervous system effects. Signs or symptoms of a bone fracture need prompt medical assessment. VITRAKVI can cause liver injury. Liver blood tests are needed before treatment, every two weeks during the first two months, and then monthly or as clinically indicated. VITRAKVI can harm an unborn baby. If you can become pregnant, use effective contraception.

Before taking this medicine
The label lists no formal contraindications for VITRAKVI. VITRAKVI can harm an unborn baby. Females who could become pregnant should use effective contraception. Do not breastfeed during treatment. Conditions requiring caution include: - Liver problems: Liver function tests are needed before treatment, every two weeks for the first two months, then monthly or as clinically indicated. Moderate or severe liver impairment requires prescriber-managed dose adjustment. - Nervous system effects: Dizziness, cognitive impairment, mood disorders and sleep disturbances can occur. Do not drive or operate hazardous machinery if you experience these effects. - Bone fractures: Signs or symptoms of a fracture require prompt medical assessment. Liver toxicity or nervous system effects may require the prescriber to interrupt, adjust or permanently stop treatment, depending on severity.
Interactions
VITRAKVI can interact with medicines that affect, or are processed by, an enzyme called CYP3A4. Ask your doctor to review your other medicines. Strong CYP3A4 inhibitors and inducers should be avoided with VITRAKVI. If their use cannot be avoided, your prescriber will need to adjust your treatment. Moderate CYP3A4 inhibitors require closer monitoring for side effects and may require a treatment adjustment. Moderate CYP3A4 inducers also require an adjustment by your prescriber. Medicines known as sensitive CYP3A4 substrates should not be taken with VITRAKVI. Your doctor can check whether any of your medicines fall into these groups.

Dosing
Dosing is set by your prescriber. Each health care professional may prescribe medication differently according to a patient's medical history and condition, so follow the directions on your prescription label and speak to your doctor or pharmacist before changing anything.
Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
