FDA boxed warningRisk of thyroid c-cell tumors

In male and female rats, dulaglutide causes a dose-related and treatment-duration-dependent increase in the incidence of thyroid C-cell tumors (adenomas and carcinomas) after lifetime exposure. It is unknown whether TRULICITY causes thyroid C-cell tumors, including medullary thyroid carcinoma (MTC), in humans as human relevance of dulaglutide-induced rodent thyroid C-cell tumors has not been determined

TRULICITY is contraindicated in patients with a personal or family history of MTC and in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Counsel patients regarding the potential risk of MTC with use of TRULICITY and inform them of symptoms of thyroid tumors (e.g., mass in the neck, dysphagia, dyspnea, persistent hoarseness). Routine monitoring of serum calcitonin or using thyroid ultrasound is of uncertain value for early detection of MTC in patients treated with TRULICITY

About Trulicity

Overview

Trulicity (dulaglutide) is a prescription GLP-1 receptor agonist supplied in a pen for injection under the skin. It is used alongside diet and exercise to improve blood sugar control in adults and children aged 10 years and older with type 2 diabetes. It is also used to reduce the risk of cardiovascular death, non-fatal heart attack or non-fatal stroke in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors. It works by increasing insulin release when blood sugar is high, reducing the release of glucagon and slowing stomach emptying.

What it treats

- To improve blood sugar control alongside diet and exercise in adults and children aged 10 years and older with type 2 diabetes. - To reduce the risk of cardiovascular death, non-fatal heart attack or non-fatal stroke in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors.

Safety information

Side effects

Common side effects of Trulicity include nausea, diarrhea, vomiting, abdominal pain and decreased appetite. Serious risks that warrant contacting a doctor include: • Acute pancreatitis (inflammation of the pancreas). Stop Trulicity if pancreatitis is suspected. • Low blood sugar, which can be severe, when used with insulin or medicines that stimulate insulin release. • Serious allergic reactions, including anaphylaxis and angioedema (swelling beneath the skin). Stop Trulicity and seek medical help promptly if a serious allergic reaction occurs. • Acute kidney injury caused by fluid loss. Your doctor may need to monitor kidney function if side effects cause dehydration. • Severe stomach or intestinal side effects. Trulicity is not recommended for people with severe gastroparesis (delayed stomach emptying). • Complications of diabetic retinopathy (diabetes-related eye disease). People with a history of this condition need monitoring. • Gallbladder problems, including gallstones or gallbladder inflammation. The label also warns about the risk of thyroid C-cell tumors. Inhalation of stomach contents into the lungs has been reported during general anesthesia or deep sedation in people receiving GLP-1 medicines. Tell your healthcare providers about Trulicity use and any planned surgery or procedure.

When to call your doctor

- A lump in your neck, trouble swallowing or breathing, or hoarseness that does not go away. - Severe stomach pain, vomiting or diarrhea.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not use Trulicity if you or anyone in your family has had medullary thyroid carcinoma (MTC), if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), or if you have had a serious allergic reaction to dulaglutide or any of its ingredients. Trulicity is not recommended for people with severe gastroparesis (severely delayed stomach emptying). During pregnancy, it should be used only if the potential benefit justifies the potential risk to the fetus. Important precautions include: • Acute pancreatitis (inflammation of the pancreas): stop Trulicity if pancreatitis is suspected. • Serious allergic reactions, including anaphylaxis and angioedema: stop Trulicity and seek medical advice promptly if a serious allergic reaction occurs. • Low blood sugar: the risk, including severe episodes, increases when used with insulin or medicines that stimulate insulin release. • Severe digestive side effects and fluid loss: kidney function may need monitoring if side effects cause fluid loss, as this can lead to acute kidney injury. • Diabetic retinopathy (diabetes-related eye damage): people with a history of this condition need monitoring. • Gallbladder problems: suspected gallstones or gallbladder inflammation require investigation. Tell your healthcare team about any planned surgery or procedure. Aspiration, stomach contents entering the lungs, has been reported during general anesthesia or deep sedation in people receiving medicines in this class.

Interactions

Trulicity slows stomach emptying, which may slow the absorption of medicines taken by mouth. Tell your doctor about any oral medicines you take.
