About Torisel

Overview

Torisel contains temsirolimus, an anticancer medicine known as an mTOR inhibitor. It is used to treat advanced renal cell carcinoma, a type of kidney cancer. It works by blocking mTOR, a protein that controls cell division, which can slow the growth of tumour cells.

Advanced renal cell carcinoma (kidney cancer).

Side effects

Common side effects include rash, weakness, inflammation or sores in the mouth, nausea, swelling and loss of appetite. Common blood test changes include anemia, low white blood cell and platelet counts, high blood sugar, increased blood fats, low phosphate, increased creatinine and raised liver enzyme levels. Contact a doctor promptly for serious side effects, including allergic or infusion reactions, infections, lung inflammation, kidney failure, protein in the urine, impaired wound healing or bleeding in the brain. Infusion reactions can be life-threatening and, rarely, fatal, including during the first infusion. Kidney failure can also be fatal. Fever, abdominal pain or bloody stools need prompt medical assessment because they may indicate a hole in the bowel. High blood sugar and high blood fats may require treatment. Older patients may be more likely to experience diarrhea, swelling and pneumonia.

When to call your doctor

• Seek medical attention immediately for fever, abdominal pain or blood in your stools. These may be signs of a hole in the bowel.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not use Torisel (temsirolimus) if your bilirubin level is more than 1.5 times the upper limit of normal. Caution is needed with mild liver impairment. Serious allergic or infusion reactions, including life-threatening and rarely fatal reactions, can occur, even early in the first infusion. Monitoring is needed throughout the infusion. Other concerns include high blood sugar or blood fats, infections, lung inflammation, bowel perforation, kidney failure, protein in the urine and impaired wound healing. Blood sugar, blood fats, kidney function and urine protein need monitoring. Extra caution is needed around surgery. Fever, abdominal pain or bloody stools need prompt medical assessment. Avoid live vaccines and close contact with people who have received them. Torisel can harm an unborn baby; use effective contraception. Do not breastfeed during treatment. Older patients may be more likely to experience diarrhoea, swelling and pneumonia.

Interactions

Medicines that strongly increase the activity of CYP3A4/5 enzymes or block CYP3A4 can affect levels of the main substance formed when the body breaks down temsirolimus. Tell your doctor about all medicines and supplements you take so they can check for these interactions. Your doctor may need to use an alternative medicine or adjust your treatment if an alternative cannot be used.
