WARNINGS
SERIOUS DERMATOLOGIC REACTIONS AND HLA-B*1502 ALLELE
SERIOUS AND SOMETIMES FATAL DERMATOLOGIC REACTIONS, INCLUDING TOXIC EPIDERMAL NECROLYSIS (TEN) AND STEVENS-JOHNSON SYNDROME (SJS), HAVE BEEN REPORTED DURING TREATMENT WITH CARBAMAZEPINE. THESE REACTIONS ARE ESTIMATED TO OCCUR IN 1 TO 6 PER 10,000 NEW USERS IN COUNTRIES WITH MAINLY CAUCASIAN POPULATIONS, BUT THE RISK IN SOME ASIAN COUNTRIES IS ESTIMATED TO BE ABOUT 10 TIMES HIGHER. STUDIES IN PATIENTS OF CHINESE ANCESTRY HAVE FOUND A STRONG ASSOCIATION BETWEEN THE RISK OF DEVELOPING SJS/TEN AND THE PRESENCE OF HLA-B*1502, AN INHERITED ALLELIC VARIANT OF THE HLA-B GENE. HLA-B*1502 IS FOUND ALMOST EXCLUSIVELY IN PATIENTS WITH ANCESTRY ACROSS BROAD AREAS OF ASIA. PATIENTS WITH ANCESTRY IN GENETICALLY AT-RISK POPULATIONS SHOULD BE SCREENED FOR THE PRESENCE OF HLA-B*1502 PRIOR TO INITIATING TREATMENT WITH CARBAMAZEPINE. PATIENTS TESTING POSITIVE FOR THE ALLELE SHOULD NOT BE TREATED WITH CARBAMAZEPINE UNLESS THE BENEFIT CLEARLY OUTWEIGHS THE RISK (SEE
AND
PRECAUTIONS, LABORATORY TESTS).
APLASTIC ANEMIA AND AGRANULOCYTOSIS
ALTHOUGH REPORTS OF TRANSIENT OR PERSISTENT DECREASED PLATELET OR WHITE BLOOD CELL COUNTS ARE NOT UNCOMMON IN ASSOCIATION WITH THE USE OF CARBAMAZEPINE, DATA ARE NOT AVAILABLE TO ESTIMATE ACCURATELY THEIR INCIDENCE OR OUTCOME. HOWEVER, THE VAST MAJORITY OF THE CASES OF LEUKOPENIA HAVE NOT PROGRESSED TO THE MORE SERIOUS CONDITIONS OF APLASTIC ANEMIA OR AGRANULOCYTOSIS.
About Tegretol
Overview
Tegretol contains carbamazepine, an anticonvulsant medicine. It is used to control seizures and relieve the nerve pain of trigeminal neuralgia. Exactly how carbamazepine works is not known.
Safety information
Side effects
The most frequently reported side effects are dizziness, drowsiness, unsteadiness, nausea and vomiting, especially when treatment begins. Other reported effects include headache, fatigue, blurred or double vision, dry mouth, abdominal discomfort, diarrhea and constipation. Contact a doctor promptly if you develop a rash, fever, chills, unusual bruising or bleeding, yellowing of the skin or eyes, difficulty passing urine or reduced urination. Serious reported reactions include severe blood cell disorders and bone marrow suppression, severe skin reactions such as Stevens–Johnson syndrome and toxic epidermal necrolysis, hepatitis, liver failure, kidney failure and inflammation of the pancreas. Seek urgent medical help for severe or peeling skin reactions, trouble breathing, fainting or collapse, or an irregular heartbeat. Heart failure, heart rhythm and conduction problems, blood clots including clots in the lungs, and inflammatory lung reactions have been reported. Some cardiovascular complications have been fatal. Low blood sodium and neuroleptic malignant syndrome have also been reported. Do not stop carbamazepine abruptly without medical guidance. In people whose epilepsy is controlled by treatment, sudden withdrawal may cause seizures or life-threatening prolonged seizures.

When to call your doctor
If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take Tegretol (carbamazepine) if you have a history of bone marrow depression, are allergic to carbamazepine, or have a known sensitivity to tricyclic medicines such as amitriptyline, desipramine, imipramine, protriptyline or nortriptyline. Do not take Tegretol with nefazodone, as carbamazepine can lower levels of nefazodone and its active metabolite enough to prevent it from working effectively. Use with monoamine oxidase (MAO) inhibitors is not recommended. MAO inhibitors must be stopped at least 14 days before starting Tegretol, or longer if the clinical situation allows.
Interactions
If you take liquid Tegretol, tell your doctor about any other liquid medicines you use, especially chlorpromazine (Thorazine) or Mellaril. Mixing these products with liquid Tegretol has been shown to form solid material. Liquid Tegretol should not be taken at the same time as other liquid medicines or liquids used to dilute medicines.
Using Tegretol
