About Diacomit
Overview
Diacomit contains stiripentol and is used to treat seizures associated with Dravet syndrome in patients taking clobazam who are at least 6 months old and weigh at least 7 kg. There are no clinical data supporting its use on its own for Dravet syndrome. Exactly how it works is unknown, but it may act on GABA receptors in the brain and increase blood levels of clobazam and its active breakdown product.
What it treats
Seizures associated with Dravet syndrome in patients aged 6 months and older who weigh at least 7 kg and are taking clobazam. There are no clinical data supporting use of Diacomit alone for Dravet syndrome.
Safety information
Side effects
Common side effects include sleepiness, reduced appetite, agitation, problems with balance or coordination, weight loss, reduced muscle tone, nausea, tremor, difficulty speaking clearly and trouble sleeping. Contact a doctor if sleepiness is excessive, particularly when Diacomit is taken with other medicines that cause sleepiness. Reduced appetite and weight loss also need medical attention; weight and children’s growth should be carefully monitored. Diacomit can lower white blood cell and platelet counts. Blood counts should be checked before treatment and every six months. Seek urgent medical help for suicidal thoughts or behaviour. Stopping Diacomit suddenly can increase seizure frequency or cause status epilepticus, a prolonged seizure emergency. Do not stop treatment without your prescriber’s guidance. Diacomit sachets contain phenylalanine, which poses a risk for people with phenylketonuria (PKU). Tell the prescriber if you have PKU so total daily phenylalanine intake can be assessed.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
The FDA label lists no contraindications for Diacomit (stiripentol). However, the following require caution: • Drowsiness, especially when taken with other medicines that slow brain activity, including clobazam. • Reduced appetite and weight loss. Weight should be monitored carefully, along with growth in children. • Low neutrophil or platelet counts. Blood counts should be checked before treatment and every six months. • Phenylketonuria (PKU): the sachets contain phenylalanine, so total daily phenylalanine intake must be considered before prescribing. • Suicidal thoughts or behaviour, which require monitoring during treatment. • Pregnancy: animal studies suggest that Diacomit may harm the unborn baby. Diacomit should be withdrawn gradually under the prescriber’s guidance to reduce the risk of more frequent seizures or status epilepticus (a prolonged seizure emergency).
Interactions
Diacomit can increase blood levels of clobazam and its breakdown product. If side effects occur, your doctor may need to reduce your clobazam dose. Other medicines may also need dose adjustments, particularly those processed by the enzymes CYP2C8, CYP2C19, CYP1A2, CYP2B6 or CYP3A4, or transported by the proteins P-gp or BCRP. Medicines that strongly increase the activity of CYP1A2, CYP3A4 or CYP2C19 may require your doctor to adjust your Diacomit dose. Ask your doctor to review your other medicines for these interactions. Do not change doses yourself.

Dosing

Storage
Store below 25°C in a dry place, in the original sachet until you use it, away from direct light and moisture. Keep all medicines out of the sight and reach of children.
