About Tarceva
Overview
Tarceva (erlotinib) is a prescription cancer medicine called a tyrosine kinase inhibitor. It treats non-small cell lung cancer that has spread locally or to other parts of the body after at least one chemotherapy regimen has failed. It is also used with gemcitabine as an initial treatment for pancreatic cancer that has spread locally, cannot be removed by surgery or has spread to other parts of the body. It blocks enzyme activity linked to the epidermal growth factor receptor (EGFR), although exactly how this produces its effect against cancer is not fully understood.
What it treats
- Locally advanced or metastatic non-small cell lung cancer after at least one previous chemotherapy regimen has failed. - First-line treatment of locally advanced, unresectable or metastatic pancreatic cancer, in combination with gemcitabine.
Safety information
Side effects
Common side effects of Tarceva include rash, diarrhoea, loss of appetite and tiredness. Nausea is also common in patients treated for pancreatic cancer. Serious side effects, including fatal cases, have been reported. Contact a doctor immediately if you develop new or worsening unexplained shortness of breath, cough or fever, as these may indicate serious lung disease. Contact a doctor promptly if you develop dehydration or bleeding. Other serious reported effects include liver failure, combined liver and kidney failure, and reduced kidney function or acute kidney failure. Heart attack or reduced blood flow to the heart, stroke, and a condition involving destruction of red blood cells with a low platelet count have been reported in patients with pancreatic cancer. Your doctor may monitor liver function and, if you are at risk of dehydration, kidney function and blood salts. Increased INR, a measure of how long blood takes to clot, and bleeding have been reported, sometimes with warfarin use. Close monitoring is needed if you take warfarin or a related anticoagulant.

Interactions
Some medicines, supplements and foods can change the amount of erlotinib in your body. Discuss the following with your doctor: • Medicines that may increase erlotinib levels, including ketoconazole, itraconazole, voriconazole, clarithromycin, ciprofloxacin, nefazodone, ritonavir and other medicines that block the enzyme CYP3A4. Grapefruit and grapefruit juice may also increase erlotinib levels. • Medicines and supplements that may reduce erlotinib levels, including rifampicin, rifabutin, rifapentine, phenytoin, carbamazepine, phenobarbital and St. John’s wort. Your doctor should consider alternatives where possible. • Medicines that reduce stomach acid. Proton pump inhibitors such as omeprazole can reduce erlotinib absorption and should be avoided with Tarceva if possible. Taking them at a different time may not prevent this interaction. Also discuss H2 blockers and antacids with your doctor; the effect of antacids on erlotinib has not been studied. • Midazolam: Tarceva may reduce the amount of midazolam in the body. Cigarette smoking reduces erlotinib levels. The label advises stopping smoking. Tell your doctor if you smoke or stop smoking during treatment, as your prescription may need to be reviewed.

Dosing

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
