About Tagrisso

Overview

Tagrisso (osimertinib) is a prescription tablet used to treat adults with certain EGFR-mutated forms of non-small cell lung cancer, identified by an FDA-approved test. Depending on the mutation and treatment history, it may be used after tumour removal, after chemoradiation for inoperable stage III cancer that has not progressed, or as initial treatment for advanced cancer, either alone or with specified chemotherapy medicines. It is also used for metastatic cancer with the EGFR T790M mutation that has progressed during or after treatment with an EGFR inhibitor. It works by blocking the activity of certain altered forms of EGFR, a protein involved in cancer cell growth.

What it treats

Tagrisso (osimertinib) is used in adults with non-small cell lung cancer (NSCLC) for: • Treatment after surgery to remove the tumour, when it has an EGFR exon 19 deletion or exon 21 L858R mutation. • Locally advanced stage III cancer that cannot be removed surgically, has an EGFR exon 19 deletion or exon 21 L858R mutation, and has not progressed during or after platinum-based chemotherapy and radiation given together or sequentially. • First-line treatment of cancer that has spread to other parts of the body and has an EGFR exon 19 deletion or exon 21 L858R mutation. • First-line treatment, together with pemetrexed and platinum-based chemotherapy, of locally advanced cancer or cancer that has spread to other parts of the body, with an EGFR exon 19 deletion or exon 21 L858R mutation. • Cancer that has spread to other parts of the body, has an EGFR T790M mutation, and has progressed during or after treatment with an EGFR tyrosine kinase inhibitor. These EGFR mutations must be detected by an FDA-approved test.

Safety information

Side effects

Common side effects of Tagrisso include diarrhea, rash, muscle or joint pain, nail problems, dry skin, mouth inflammation and tiredness. Blood tests may show low white blood cell, platelet or red blood cell counts. The most common effects vary with the treatment used: after platinum-based chemotherapy and radiation, lung inflammation, cough and COVID-19 were also commonly reported. When Tagrisso is combined with pemetrexed and platinum-based chemotherapy, increased blood creatinine was also common. Contact your doctor promptly about new or worsening breathing symptoms, which may indicate serious lung inflammation (interstitial lung disease or pneumonitis). Other serious side effects include changes in the heart’s electrical activity (QT prolongation), heart muscle problems (cardiomyopathy), inflammation of the cornea (keratitis), inflammation of blood vessels in the skin, and aplastic anemia, a condition in which the bone marrow does not make enough blood cells. Serious skin reactions include erythema multiforme major, Stevens-Johnson syndrome and toxic epidermal necrolysis. Contact your doctor promptly if you develop eye symptoms or a new or worsening skin reaction. Some of these effects require heart monitoring, blood tests, specialist assessment or stopping treatment under medical supervision.

When to call your doctor

• New or worsening cough or breathing problems, seek medical attention immediately. • A severe skin rash, seek medical attention immediately. • Eye inflammation, seek prompt medical attention for an eye specialist’s assessment.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

The FDA label lists no contraindications for Tagrisso. However, important precautions apply: • Lung inflammation (interstitial lung disease/pneumonitis): New or worsening breathing symptoms need medical assessment. Treatment may need to be interrupted or permanently stopped, depending on severity and whether you recently received definitive platinum-based chemoradiation. • Heart conditions: Tagrisso can prolong the QTc interval, affecting heart rhythm, and can weaken the heart muscle. People with a history of or risk factors for QTc prolongation, or who take medicines that prolong QTc, need ECG and electrolyte monitoring. Heart function monitoring is needed for people with cardiac risk factors. • Eye inflammation (keratitis): Symptoms require prompt assessment by an eye specialist. • Serious skin reactions: Treatment must be paused if erythema multiforme major, Stevens–Johnson syndrome or toxic epidermal necrolysis is suspected, and permanently stopped if confirmed. Suspected inflammation of skin blood vessels (cutaneous vasculitis) also requires a treatment pause and assessment. • Aplastic anemia: Treatment must be paused if this condition is suspected and permanently stopped if confirmed. • Pregnancy: Tagrisso can harm an unborn baby. Females who could become pregnant should use effective contraception during treatment and for 6 weeks after the last dose. Males should use effective contraception for 4 months after the last dose. • Breastfeeding: Do not breastfeed.

Interactions

Avoid taking Tagrisso with medicines that strongly increase the activity of the enzyme CYP3A (strong CYP3A inducers). Tell your doctor about all medicines you take so they can check for this interaction. If the combination cannot be avoided, your prescriber will need to adjust your Tagrisso dose.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
