About Tafinlar

Overview

Tafinlar capsules contain dabrafenib, a cancer medicine called a kinase inhibitor. Its uses include melanoma that cannot be removed by surgery or has spread, either alone for cancers with a BRAF V600E mutation or with trametinib for cancers with BRAF V600E or V600K mutations. With trametinib, it also treats non-small cell lung cancer that has spread and anaplastic thyroid cancer that is locally advanced or has spread when there are no satisfactory local treatment options; both must have a BRAF V600E mutation. It works by blocking certain altered BRAF proteins that can drive cancer cell growth.

What it treats

- Melanoma that cannot be removed by surgery or has spread: used alone for tumors with a BRAF V600E mutation, or with trametinib for tumors with BRAF V600E or V600K mutations. - Melanoma with a BRAF V600E or V600K mutation and lymph node involvement: used with trametinib after complete surgical removal. - Metastatic non-small cell lung cancer with a BRAF V600E mutation: used with trametinib. - Anaplastic thyroid cancer with a BRAF V600E mutation that is locally advanced or has spread, when there are no satisfactory local or regional treatment options: used with trametinib. - Solid tumors with a BRAF V600E mutation that cannot be removed by surgery or have spread, in adults and children aged 1 year and older, after progression on prior treatment and when no satisfactory alternatives remain: used with trametinib. This indication has accelerated approval; continued approval may depend on confirmatory trials. - Low-grade glioma with a BRAF V600E mutation in children aged 1 year and older who need systemic treatment: used with trametinib. Tafinlar is not indicated for colorectal cancer or solid tumors without a BRAF mutation.

Safety information

Side effects

Common side effects of Tafinlar taken alone include thickened skin, headache, fever, joint pain, wart-like skin growths, hair loss, and redness, swelling or pain affecting the palms and soles. When Tafinlar is taken with trametinib, common side effects vary by the condition treated and age group. They include fever, chills, rash, tiredness, headache, nausea, vomiting, diarrhea, cough, dry skin, reduced appetite, swelling, bleeding, shortness of breath, constipation, abdominal pain, and muscle or joint pain. Children may also develop an acne-like rash or inflammation around the nails. Contact a doctor promptly about bleeding, fever, severe or persistent skin reactions, or any changes in vision. Serious risks include major bleeding, weakening of the heart muscle, eye inflammation, serious fever reactions, severe skin reactions and high blood sugar. Fever can be more frequent and more severe when Tafinlar is taken with trametinib. People with G6PD deficiency need close monitoring for anemia caused by the breakdown of red blood cells. Other serious risks include new cancers, including skin cancers, increased growth of tumors without a BRAF mutation, and hemophagocytic lymphohistiocytosis (HLH). Monitoring for new cancers is needed before, during and after treatment. Heart function monitoring is needed with combination treatment, and blood sugar monitoring is needed in people with existing diabetes or high blood sugar. Tafinlar can harm an unborn baby. Those who could become pregnant should use effective non-hormonal contraception.

When to call your doctor

Seek medical attention immediately for: • Unusual or heavy bleeding. • Changes in your vision. • A high fever or fever with severe chills. • A severe or worsening skin rash. • Shortness of breath.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

The FDA label lists no formal contraindications for Tafinlar. However, important precautions apply. Tafinlar can cause new skin cancers and other cancers, whether used alone or with trametinib. Monitoring is needed before, during and after treatment. It may also promote tumour growth in tumours without a BRAF mutation. Other risks include eye inflammation, serious fever reactions, severe skin reactions and haemophagocytic lymphohistiocytosis (HLH). Visual changes need an eye assessment. When used with trametinib, major bleeding and heart muscle problems can occur; monitoring for bleeding and regular heart function checks are needed. People with diabetes or existing high blood sugar need blood glucose monitoring. Those with glucose-6-phosphate dehydrogenase (G6PD) deficiency need close monitoring for haemolytic anaemia. Tafinlar can harm an unborn baby. Females who could become pregnant should use effective non-hormonal contraception. Do not breastfeed during treatment. Tafinlar may impair fertility in females and males.

Interactions

Strong inhibitors of the enzymes CYP3A4 or CYP2C8 should not be taken with Tafinlar. Tafinlar may also reduce the effectiveness of medicines that rely on CYP3A4, CYP2C8, CYP2C9, CYP2C19 or CYP2B6 for processing in the body. Ask your doctor to check whether any of your medicines are affected.

Dosing

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
