Test all patients for evidence of current or prior hepatitis B virus (HBV) infection before initiating treatment with SOVALDI. HBV reactivation has been reported in HCV/HBV coinfected patients who were undergoing or had completed treatment with HCV direct acting antivirals and were not receiving HBV antiviral therapy. Some cases have resulted in fulminant hepatitis, hepatic failure, and death. Monitor HCV/HBV coinfected patients for hepatitis flare or HBV reactivation during HCV treatment and post-treatment follow-up. Initiate appropriate patient management for HBV infection as clinically indicated.

About Sovaldi
Overview
Sovaldi contains sofosbuvir, a direct-acting antiviral medicine used with other antiviral medicines to treat chronic hepatitis C. It is indicated for adults with hepatitis C genotypes 1, 2, 3 or 4, and with ribavirin for children aged 3 years and older with genotypes 2 or 3. These uses include people without cirrhosis or with compensated cirrhosis, where the scarred liver still functions. Sofosbuvir works by blocking an enzyme the hepatitis C virus needs to make copies of itself.
What it treats
- Chronic hepatitis C, genotypes 1, 2, 3 or 4, in adults without cirrhosis or with compensated cirrhosis, as part of combination antiviral treatment. - Chronic hepatitis C, genotypes 2 or 3, in children aged 3 years and older without cirrhosis or with compensated cirrhosis, in combination with ribavirin.
Safety information
Side effects
Common side effects of Sovaldi taken with ribavirin include tiredness and headache. When taken with peginterferon alfa and ribavirin, common side effects include tiredness, headache, nausea, trouble sleeping and anemia (low red blood cell levels). Decreased appetite was the most common side effect reported in children taking Sovaldi with ribavirin. Serious risks that need medical attention include: • A seriously slow heartbeat with symptoms when Sovaldi is taken with amiodarone. This combination is not recommended. The risk is particularly concerning in people also taking beta blockers or who have heart conditions or advanced liver disease. Contact your doctor promptly if you take amiodarone; heart monitoring is recommended if there is no suitable treatment alternative. • Reactivation of hepatitis B, which can cause a flare of hepatitis. All patients should be tested for current or past hepatitis B before treatment. People with both hepatitis B and hepatitis C need monitoring during treatment and follow-up. Contact your doctor if you have current or past hepatitis B so appropriate monitoring and care can be arranged.

Before taking this medicine
When Sovaldi is used with ribavirin, or with peginterferon alfa and ribavirin, anyone who must not take those medicines must not take the combination. The prescribing information for those medicines lists the applicable contraindications. All patients should be tested for current or past hepatitis B infection before starting hepatitis C treatment. Hepatitis B can reactivate during treatment. Patients with both infections need monitoring for reactivation and worsening hepatitis during treatment and follow-up. Taking Sovaldi with amiodarone is not recommended because a seriously slow heartbeat with symptoms may occur. The risk is particularly important in people also taking beta blockers or who have existing heart conditions or advanced liver disease. If there are no suitable alternative treatments, heart monitoring is recommended.
Interactions
Tell your doctor about all medicines and supplements you take, especially: • Amiodarone: taking it with a treatment regimen containing Sovaldi may cause a serious, abnormally slow heartbeat with symptoms. • Rifampin and St. John’s wort: these can change sofosbuvir levels in the body. As hepatitis C clears during treatment, liver function may change and affect how other medicines work. Your doctor may need to monitor blood clotting tests (INR) or blood glucose more frequently and adjust certain other medicines.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
