FDA boxed warningHepatotoxicity

Hepatotoxicity has been reported with use of SELZENTRY. Severe rash or evidence of a systemic allergic reaction (e.g., fever, eosinophilia, or elevated IgE) prior to the development of hepatotoxicity may occur. Patients with signs or symptoms of hepatitis or allergic reaction following use of SELZENTRY should be evaluated immediately.

Hepatotoxicity has been reported which may be preceded by severe rash or other features of a systemic allergic reaction (e.g., fever, eosinophilia, or elevated IgE).

Immediately evaluate patients with signs or symptoms of hepatitis or allergic reaction.

About Selzentry

Overview

Selzentry contains maraviroc, an HIV-1 antiviral medicine supplied as tablets. It is used with other antiretroviral medicines to treat only CCR5-tropic HIV-1 infection. Maraviroc blocks the interaction between HIV-1 and a protein called CCR5, preventing this type of HIV-1 from entering cells. Selzentry is not recommended for dual/mixed- or CXCR4-tropic HIV-1.

What it treats

CCR5-tropic HIV-1 infection in adults and children weighing at least 2 kg, used with other antiretroviral medicines. Not recommended for dual/mixed- or CXCR4-tropic HIV-1.

Safety information

Side effects

Common side effects reported in adults include upper respiratory tract infections, cough, fever, rash, dizziness, bronchitis, gas, bloating, and slowed movement of the stomach or intestines. Common side effects in children who had previously received treatment include vomiting, abdominal pain, diarrhoea, nausea, and dizziness. Serious reactions include liver injury, sometimes accompanied by a severe rash or a potentially life-threatening allergic reaction. Contact a doctor promptly if a rash or signs of liver inflammation or an allergic reaction develop. Liver blood tests are needed before treatment and may be needed during treatment. Severe, potentially life-threatening skin and allergic reactions, including Stevens–Johnson syndrome and toxic epidermal necrolysis, have been reported. Stop taking Selzentry and seek immediate medical attention if signs of a severe skin or allergic reaction occur. Heart-related events, including reduced blood flow to the heart and heart attack, were observed more often in previously treated adults receiving Selzentry. Seek immediate medical attention for suspected heart-related symptoms. A drop in blood pressure on standing is also a concern, particularly in people with severe kidney impairment or end-stage kidney disease; contact a doctor if this occurs.

When to call your doctor

• Seek medical attention immediately for a severe rash, or a rash with fever. These may signal a serious allergic reaction or liver injury.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Selzentry if you have severe kidney impairment or end-stage kidney disease and also take medicines that strongly inhibit or induce the CYP3A enzyme. Selzentry can cause liver injury, sometimes with a severe rash or a potentially life-threatening allergic reaction. Liver blood tests are needed before treatment and during treatment when clinically indicated. Extra monitoring may be needed if you already have liver problems or hepatitis B or C. Severe, potentially life-threatening skin and allergic reactions have been reported. Stop taking Selzentry and seek urgent medical attention if signs of a severe skin or allergic reaction develop. More cardiovascular events, including reduced blood flow to the heart and heart attacks, were observed in people receiving Selzentry who had previously received HIV treatment. Additional monitoring may be needed. If you have severe kidney impairment or end-stage kidney disease, tell your prescriber if you develop low blood pressure when standing, as treatment may need adjustment. Women with HIV should not breastfeed because HIV may be transmitted to the baby.

Interactions

Medicines that block the CYP3A enzyme, including protease inhibitors other than tipranavir/ritonavir, can increase Selzentry levels in the body. Medicines that increase CYP3A activity, including efavirenz, may lower Selzentry levels. Discuss these medicines with your doctor. Taking St. John’s wort with Selzentry is not recommended. Tell your doctor if you use this supplement.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
