About Sanctura

Overview

Sanctura contains trospium chloride, a medicine known as a muscarinic antagonist. It is used to treat overactive bladder symptoms, including sudden urges to urinate, frequent urination and leakage associated with urgency. It works by blocking the effects of a chemical messenger called acetylcholine on receptors in the bladder, helping the bladder muscle relax.

Overactive bladder with symptoms of a sudden, urgent need to urinate, frequent urination, and urine leakage associated with urgency.

Side effects

Common side effects of trospium chloride include dry mouth, constipation and headache. Other reported effects include upper stomach pain, indigestion, gas, tiredness, dry eyes and difficulty emptying the bladder. Blurred vision, vomiting, altered taste, dry throat, dry skin and a fast heartbeat have also been reported. Stop taking the medicine and seek emergency medical help if you develop swelling of the face, lips, tongue or throat, especially with difficulty breathing. This can occur after the first dose and may be life-threatening. Severe allergic reactions and Stevens–Johnson syndrome, a serious skin reaction, have also been reported. Contact a doctor promptly if you cannot empty your bladder, have severe or worsening constipation or stomach symptoms, or develop confusion, hallucinations or marked drowsiness. Seek urgent medical attention for chest pain, fainting, a rapid or irregular heartbeat, or a severe skin rash. Dizziness and sleepiness can occur. Do not drive or operate heavy machinery until you know how the medicine affects you. People with moderate kidney impairment are expected to have a higher risk of certain side effects, including dry mouth, constipation, indigestion, urinary tract infection and difficulty emptying the bladder. Some serious reactions were reported after approval; their frequency and whether trospium chloride caused them cannot always be determined.

When to call your doctor

- Seek emergency help for swelling of the face, lips, tongue or throat, especially with trouble breathing. - Get medical attention immediately if you cannot pass urine. - Seek immediate medical attention for chest pain, fainting or a very fast heartbeat. - Get medical attention immediately for new confusion or seeing or hearing things that are not there.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take trospium chloride if you have, or are at risk of, urinary retention (inability to empty the bladder), gastric retention (the stomach not emptying properly), or uncontrolled narrow-angle glaucoma. Do not take it if you are allergic to trospium chloride or any of its ingredients. Tell your prescriber if you have a blockage affecting urine flow, a stomach or intestinal obstruction, ulcerative colitis, reduced intestinal movement, or myasthenia gravis. Trospium chloride can slow movement through the digestive tract and increase the risk of urinary or gastric retention. If your narrow-angle glaucoma is controlled with treatment, use requires careful monitoring and a judgement that the benefits outweigh the risks. Moderate kidney impairment may increase the risk of side effects, including dry mouth, constipation, indigestion, urinary tract infection and urinary retention. Trospium chloride can cause dizziness, confusion, hallucinations or sleepiness. Do not drive or operate heavy machinery until you know how it affects you. Tell your prescriber if these effects occur, particularly when starting treatment or after a dose increase. Swelling of the face, lips, tongue or throat can occur, including after the first dose. Swelling affecting the airway can be life-threatening. Stop taking the medicine and seek urgent medical help if tongue or throat swelling occurs. There are no adequate, well-controlled studies in pregnant women. Use during pregnancy should be considered only if the potential benefit outweighs the risks to the patient and fetus. Contact your prescriber if you become pregnant during treatment. Safety and effectiveness in children have not been established.

Interactions

Trospium chloride may interact with medicines that use the same kidney clearance pathway, including procainamide, pancuronium, morphine, vancomycin and tenofovir. Taking them together may raise blood levels of trospium or the other medicine. Careful monitoring is recommended. Other antimuscarinic medicines may make dry mouth, constipation and similar side effects more frequent or severe. Trospium may also change how some medicines are absorbed by affecting movement through the stomach and intestines. Immediate-release metformin reduced trospium exposure when taken with Sanctura XR extended-release capsules. Tell your doctor if you take metformin. A study found that taking trospium with digoxin did not change how either medicine was processed by the body.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
