About Rydapt

Overview

Rydapt (midostaurin) is a kinase inhibitor supplied as capsules. It is used in adults with newly diagnosed acute myeloid leukemia (AML) that tests positive for an FLT3 mutation using an FDA-approved test, alongside cytarabine and daunorubicin chemotherapy, not on its own to start AML treatment. It also treats adults with aggressive systemic mastocytosis, systemic mastocytosis with an associated blood cancer, or mast cell leukemia. It works by blocking several proteins involved in cell signaling, which can inhibit the growth of leukemia cells and mast cells.

What it treats

- Newly diagnosed acute myeloid leukemia (AML) in adults with an FLT3 mutation confirmed by an FDA-approved test. Used with cytarabine and daunorubicin during induction, and with cytarabine during consolidation, not alone for induction. - Aggressive systemic mastocytosis (ASM) in adults. - Systemic mastocytosis with an associated hematological neoplasm (SM-AHN) in adults. - Mast cell leukemia (MCL) in adults.

Side effects

Common side effects of Rydapt (midostaurin) vary with the condition being treated: In acute myeloid leukemia (AML), when used with chemotherapy, common side effects include nausea, vomiting, inflammation or sores in the mouth, headache, muscle or bone pain, nosebleeds, small spots of bleeding under the skin, upper respiratory infections, infections related to medical devices, high blood sugar, fever with low white blood cell counts, and changes in the heart’s electrical activity called QT prolongation. In systemic mastocytosis, common side effects include nausea, vomiting, diarrhea, constipation, swelling, muscle or bone pain, abdominal pain, tiredness, upper respiratory infections, fever, headache and shortness of breath. Serious side effects include lung inflammation or damage, which can be fatal; severe infections, including sepsis and pneumonia; fever with low white blood cell counts; bleeding in the digestive tract; kidney problems; and QT prolongation. Contact a doctor promptly if you develop new or worsening breathing problems, fever, signs of infection or unusual bleeding. The label directs that Rydapt be stopped if signs or symptoms of lung toxicity occur. Rydapt may also harm an unborn baby if taken during pregnancy. Contact your doctor if you are pregnant or think you may be pregnant.

When to call your doctor

- Seek immediate medical attention for new or worsening shortness of breath or cough, which may signal serious lung inflammation. - Fever, which may signal an infection or occur with a low white blood cell count. - Unusual bleeding, including nosebleeds or new pinpoint red or purple spots on the skin. - Severe abdominal pain, vomiting or diarrhea.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Rydapt if you are allergic to midostaurin or any of its other ingredients. Severe allergic reactions have included anaphylactic shock, difficulty breathing, flushing, chest pain, and swelling of the airways or tongue. Rydapt may harm an unborn baby if taken during pregnancy. Do not breastfeed during treatment. Rydapt can cause serious lung inflammation, including interstitial lung disease or pneumonitis. Fatal cases have occurred. Patients should be monitored for symptoms, and treatment must be stopped if signs or symptoms of lung toxicity develop.

Interactions

Tell your doctor about all medicines and supplements you take. Strong inhibitors of the enzyme CYP3A4 can increase levels of midostaurin and its active breakdown products, raising the risk of side effects. Your doctor may consider alternatives or monitor you more closely. Strong CYP3A4 inducers can lower these levels and should not be used with Rydapt. Your doctor may also need to adjust other medicines processed by CYP2B6 or transported by BCRP or OATP1B1 when they are taken with Rydapt.

Dosing

Dosing is set by your prescriber. Each health care professional may prescribe medication differently according to a patient's medical history and condition, so follow the directions on your prescription label and speak to your doctor or pharmacist before changing anything.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
