About Retevmo
Overview
Retevmo (selpercatinib) is a kinase inhibitor used to treat locally advanced or metastatic non-small cell lung cancer in adults whose cancer has a RET gene fusion confirmed by an FDA-approved test. It also treats adults and children aged 2 years and older with advanced or metastatic medullary thyroid cancer with a RET mutation, or thyroid cancer with a RET gene fusion, confirmed by an FDA-approved test, when treatment throughout the body is needed; for RET fusion-positive thyroid cancer, radioactive iodine must no longer work if it is appropriate. It is also used in adults and children aged 2 years and older with locally advanced or metastatic solid tumors with an FDA-test-confirmed RET gene fusion that have progressed during or after previous systemic treatment, or when there are no satisfactory alternative treatments. It works by blocking RET proteins whose abnormal activity can drive cancer cell growth.
What it treats
- Locally advanced or metastatic non-small cell lung cancer with a RET gene fusion in adults. - Advanced or metastatic medullary thyroid cancer with a RET mutation in adults and children aged 2 years and older who need systemic therapy. - Advanced or metastatic thyroid cancer with a RET gene fusion in adults and children aged 2 years and older who need systemic therapy and whose cancer does not respond to radioactive iodine, if radioactive iodine is appropriate. - Locally advanced or metastatic solid tumors with a RET gene fusion in adults and children aged 2 years and older whose cancer has progressed during or after prior systemic treatment, or who have no satisfactory alternative treatment options. For each use, the RET gene fusion or mutation must be detected by an FDA-approved test.
Safety information
Side effects
Common side effects in adults include muscle, bone or joint pain, swelling, diarrhea, tiredness, dry mouth, high blood pressure, abdominal pain, rash, nausea, constipation, cough, headache, vomiting, shortness of breath and bleeding. In children, common side effects include muscle, bone or joint pain, diarrhea, nausea, bleeding, fever, abdominal pain, headache, vomiting, tiredness, cough, rash, coronavirus infection, upper respiratory tract infection and swelling. Serious side effects that warrant contacting a doctor include liver injury, lung inflammation, high blood pressure, changes in the heart’s electrical rhythm (QT prolongation), severe or life-threatening bleeding, hypersensitivity reactions, tumor lysis syndrome (complications from rapid breakdown of tumor cells), impaired wound healing and an underactive thyroid. Slipping of the growth plate at the top of the thigh bone is also a serious risk in children and adolescents. Contact a doctor promptly for new or worsening breathing symptoms, bleeding or a suspected hypersensitivity reaction. Severe bleeding needs urgent medical attention. Blood tests may show severe changes in liver enzymes, blood cell counts and mineral levels. Monitoring includes liver and thyroid function, blood pressure and, in patients at significant risk of QT prolongation, heart rhythm and electrolytes. Retevmo can also harm an unborn baby; effective contraception is advised for females who could become pregnant.

Before taking this medicine
The FDA label lists no contraindications for Retevmo (selpercatinib). However, treatment should not start if high blood pressure is uncontrolled. Important risks requiring caution and monitoring include liver injury, lung inflammation, high blood pressure, heart rhythm changes called QT prolongation, severe bleeding, hypersensitivity reactions, tumour lysis syndrome, impaired wound healing and an underactive thyroid. Monitoring includes liver tests, blood pressure, thyroid function and, in patients at significant risk of QT prolongation, heart rhythm and electrolyte levels. More frequent heart rhythm monitoring is needed with certain interacting medicines. Treatment must be permanently stopped if severe or life-threatening bleeding occurs. Other serious reactions may require treatment to be paused, adjusted or stopped by the prescriber. Retevmo must also be paused around planned surgery because it can impair wound healing. Retevmo can harm an unborn baby. Females who could become pregnant should use effective contraception. Do not breastfeed during treatment. Children require monitoring of open growth plates and for signs of a hip growth-plate disorder called slipped capital femoral epiphysis. Treatment may need to be interrupted or stopped if growth-plate abnormalities occur.
Interactions
Retevmo can interact with the following medicines: • Acid-reducing medicines, including proton pump inhibitors (PPIs), H2 blockers and locally acting antacids. Avoid taking these with Retevmo where possible. If a PPI cannot be avoided, take Retevmo with food. For H2 blockers or antacids, ask your doctor how to separate administration times. • Strong or moderate CYP3A inhibitors or inducers. These combinations should be avoided. If a CYP3A inhibitor is necessary, your prescriber will need to adjust your Retevmo dose. • Medicines processed by CYP2C8 or CYP3A, and certain medicines transported by P-gp or BCRP. These combinations should be avoided; if necessary, your prescriber may need to adjust the other medicine. Ask your doctor or pharmacist to check your medicines for these interactions. Do not change doses yourself.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
