About Prograf

Overview

Prograf capsules contain tacrolimus, an immunosuppressant medicine. It is used with other immunosuppressants to help prevent organ rejection in adults and children who receive a liver, kidney, heart or lung transplant. It works by blocking calcineurin, an enzyme involved in activating immune cells, which reduces the immune response against the transplanted organ.

What it treats

Prevention of organ rejection in adults and children receiving liver, kidney, heart or lung transplants, used in combination with other medicines that suppress the immune system.

Side effects

Common side effects of Prograf (tacrolimus) include changes in kidney function, high blood pressure, diabetes or high blood sugar, fever, infections (including cytomegalovirus, bronchitis and urinary tract infections), low white blood cell counts and anemia. Other common effects include shaking, headache, trouble sleeping, tingling or numbness, swelling of the arms or legs, nausea, abdominal pain, constipation and diarrhea. High potassium, low magnesium, high blood fat levels and fluid buildup around the heart are also listed as common side effects. Serious side effects warrant prompt contact with a doctor. These include serious infections, lymphoma and other cancers, new diabetes after transplantation, sudden or long-term kidney damage, and effects on the brain or nervous system, including posterior reversible encephalopathy syndrome (PRES). Other serious risks include high potassium, high blood pressure, thickening of the heart muscle, and pure red cell aplasia, a condition in which the body does not make enough red blood cells. Prograf can also cause thrombotic microangiopathy, a serious condition involving clots in small blood vessels, including hemolytic uremic syndrome and thrombotic thrombocytopenic purpura. Some of these problems are detected through monitoring rather than noticeable symptoms. The label calls for checks of blood glucose and potassium levels and monitoring for nervous system abnormalities.

When to call your doctor

• Seek medical attention immediately for fever, as Prograf increases the risk of serious infections. • Get prompt medical attention for new or worsening shaking, headache or tingling, which may indicate effects on the nervous system.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Prograf capsules if you are allergic to tacrolimus. Reported allergic reactions include rash, itching and difficulty breathing. Prograf requires careful monitoring because it can cause kidney damage, new diabetes after transplantation, high potassium levels, high blood pressure and nervous system problems, including posterior reversible encephalopathy syndrome (PRES). Blood glucose and potassium levels should be monitored. Extra caution is needed with other medicines that can damage the kidneys or raise potassium levels. Other serious risks include thickening of the heart muscle, pure red cell aplasia (a condition in which the body stops making enough red blood cells), and thrombotic microangiopathy (damage and clots in small blood vessels). The risk of thrombotic microangiopathy may be greater with infections or certain other medicines. Use with sirolimus is not recommended in liver or heart transplant patients because of an increased risk of serious adverse reactions. Avoid live vaccines during treatment. Prograf can harm an unborn baby. If you are pregnant, discuss the potential risks with your prescriber.

Interactions

Avoid grapefruit juice and taking nelfinavir with Prograf, as they can increase tacrolimus levels. Tell your doctor about all medicines and supplements you take, including cannabidiol (CBD). Medicines that block CYP3A can raise tacrolimus levels, while medicines that increase CYP3A4 activity can lower them. Your doctor may need to monitor tacrolimus blood levels and adjust treatment. CBD may also require monitoring and a reduction in your prescribed Prograf dose. If you take a mycophenolic acid product, including mycophenolate mofetil, switching from cyclosporine to Prograf can increase mycophenolic acid exposure. Your doctor may monitor for related side effects and adjust that treatment.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
