About Piqray
Overview
Piqray (alpelisib) is a kinase inhibitor supplied as tablets. It is used with fulvestrant to treat adults with advanced or metastatic breast cancer that is hormone receptor (HR)-positive, HER2-negative and has a PIK3CA mutation detected by an FDA-approved test. It is used after the cancer has progressed during or after endocrine-based treatment. Piqray works by blocking PI3K, mainly its alpha form, which is involved in signals that promote cancer cell growth.
What it treats
Used with fulvestrant to treat adults with advanced or metastatic breast cancer that is hormone receptor (HR)-positive, HER2-negative and has a PIK3CA mutation detected by an FDA-approved test, after the cancer has progressed during or after endocrine-based treatment.
Safety information
Side effects
Common side effects of Piqray include diarrhea, rash, nausea, vomiting, tiredness, reduced appetite, mouth inflammation or sores, weight loss and hair loss. Common blood test changes include high or low blood sugar, increased creatinine, liver enzymes and lipase, reduced lymphocytes (a type of white blood cell), hemoglobin and calcium, and a longer blood-clotting test time. Serious side effects include severe allergic reactions, severe skin reactions, severely high blood sugar, lung inflammation and inflammation of the colon (colitis). Seek urgent medical attention for signs of a severe allergic or skin reaction. Severely high blood sugar can lead to serious complications, including ketoacidosis; blood sugar testing is needed before and during treatment. Diarrhea can be severe and cause dehydration and acute kidney injury. If diarrhea occurs, start antidiarrheal treatment, increase oral fluids and notify your healthcare provider. Contact your doctor promptly if you develop abdominal pain or mucus or blood in your stool, which may indicate colitis. Piqray can harm an unborn baby. Effective contraception is needed during treatment.

Before taking this medicine
Do not take Piqray if you have had a severe allergic reaction to alpelisib or any of its ingredients. Piqray can cause severe skin reactions, very high blood sugar, lung inflammation, and severe diarrhea or inflammation of the colon (colitis). Blood glucose and HbA1c should be checked before treatment and monitored during treatment. Blood glucose should be brought under control before starting Piqray. Severe allergic or skin reactions require prompt medical attention. Tell your healthcare provider if diarrhea occurs, or if you develop abdominal pain or mucus or blood in your stool. Severe diarrhea can cause dehydration and acute kidney injury. Serious side effects may require treatment to be interrupted or permanently stopped. Piqray can harm an unborn baby. Use effective contraception during treatment. Do not breastfeed while taking Piqray.
Interactions
Piqray should not be used with medicines that strongly increase the activity of an enzyme called CYP3A4, or with medicines that block a protein called BCRP. Ask your doctor to review your other medicines and consider alternatives. If a BCRP-blocking medicine cannot be replaced, your doctor should monitor you closely for increased side effects.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
