About Pifeltro
Overview
Pifeltro is a prescription tablet containing doravirine, an antiretroviral medicine. It is used with other antiretroviral medicines to treat HIV-1 infection in adults and children weighing at least 35 kg. It is for people who have not previously received antiretroviral treatment, or those switching from stable treatment whose HIV-1 is suppressed, with no history of treatment failure and no known virus changes linked to doravirine resistance. Doravirine works by blocking an HIV-1 enzyme called reverse transcriptase.
What it treats
Treatment of HIV-1 infection, in combination with other antiretroviral medicines, in adults and children weighing at least 35 kg who: • Have not previously received antiretroviral treatment; or • Are switching from a stable antiretroviral regimen, have HIV-1 RNA below 50 copies/mL, and have no history of treatment failure or known viral changes associated with resistance to doravirine.
Safety information
Side effects
Common side effects include nausea, dizziness, headache, tiredness, diarrhea, abdominal pain and abnormal dreams. Severe skin reactions have been reported, including Stevens–Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), and Drug Rash with Eosinophilia and Systemic Symptoms (DRESS). Stop taking Pifeltro immediately and seek urgent medical advice if signs or symptoms of a severe skin reaction develop. The label also warns about immune reconstitution syndrome, which requires monitoring by your doctor.
When to call your doctor
If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take Pifeltro with medicines that strongly increase the activity of the CYP3A enzyme, as these can lower doravirine levels and reduce its effectiveness. These include carbamazepine, oxcarbazepine, phenobarbital, phenytoin, enzalutamide, rifampin, rifapentine, mitotane and St. John’s wort. This is not a complete list. Severe skin reactions, including Stevens–Johnson syndrome, toxic epidermal necrolysis and DRESS (drug rash with eosinophilia and systemic symptoms), have been reported. Stop Pifeltro immediately if signs or symptoms of a severe skin reaction develop. Monitoring for immune reconstitution syndrome is required. Pifeltro is not recommended for children weighing less than 35 kg.
