Cardiomyopathy: PHESGO administration can result in subclinical and clinical cardiac failure manifesting as CHF, and decreased LVEF, with greatest risk when administered concurrently with anthracyclines. Evaluate cardiac function prior to and during treatment. Discontinue PHESGO for cardiomyopathy.
Embryo-fetal Toxicity: Exposure to PHESGO can result in embryo-fetal death and birth defects. Advise patients of these risks and the need for effective contraception.
Pulmonary Toxicity: Discontinue PHESGO for anaphylaxis, angioedema, interstitial pneumonitis, or acute respiratory distress syndrome.
About PHESGO
Overview
PHESGO combines pertuzumab, trastuzumab and hyaluronidase-zzxf in a vial for injection under the skin. It is used with chemotherapy before surgery for HER2-positive locally advanced, inflammatory or early-stage breast cancer when the tumour is larger than 2 cm or cancer involves lymph nodes, or after surgery for HER2-positive early breast cancer at high risk of returning. It is also used with docetaxel for HER2-positive breast cancer that has spread to other parts of the body, when no previous anti-HER2 treatment or chemotherapy has been given for metastatic disease. Pertuzumab and trastuzumab bind to HER2 to block signals that promote cancer cell growth, while hyaluronidase helps the medicines spread and be absorbed under the skin.
What it treats
- With chemotherapy before surgery for HER2-positive locally advanced, inflammatory or early-stage breast cancer that is larger than 2 cm or has spread to lymph nodes, as part of a complete treatment regimen. - With chemotherapy after surgery for HER2-positive early breast cancer at high risk of returning. - With docetaxel for HER2-positive metastatic breast cancer in patients who have not previously received anti-HER2 therapy or chemotherapy for metastatic disease.
Safety information
Side effects
Common side effects reported with PHESGO during breast cancer treatment before or after surgery include hair loss, nausea, diarrhea, anemia (low red blood cell count), and weakness. Serious side effects warranting prompt medical attention include heart muscle problems, lung toxicity, worsening of chemotherapy-related neutropenia (low levels of a type of white blood cell), and allergic or administration-related reactions. Severe allergic reactions, including anaphylaxis, require emergency medical care and permanent discontinuation of PHESGO. PHESGO can harm an unborn baby. Contact your doctor promptly if you are pregnant or think you may be pregnant.

Before taking this medicine
Do not use PHESGO if you have a known allergy to pertuzumab, trastuzumab, hyaluronidase or any of its other ingredients. PHESGO may worsen low neutrophil counts caused by chemotherapy. Patients should be monitored for allergic reactions during treatment. PHESGO must be permanently stopped if anaphylaxis or a severe allergic reaction occurs. Pregnancy status should be checked before starting PHESGO in patients who could become pregnant.
Interactions
Receiving anthracycline chemotherapy after stopping PHESGO may increase the risk of heart problems because PHESGO takes a long time to clear from the body. Tell your doctor if anthracycline treatment is planned. If possible, it should be avoided for up to 7 months after stopping PHESGO. If anthracyclines are used, your heart function should be carefully monitored.
