About Pemazyre
Overview
Pemazyre (pemigatinib) is a kinase inhibitor supplied as tablets. It treats adults with previously treated bile duct cancer (cholangiocarcinoma) that cannot be removed surgically, is locally advanced or has spread, and has an FGFR2 fusion or other rearrangement detected by an FDA-approved test. It also treats adults with myeloid/lymphoid neoplasms with an FGFR1 rearrangement that have returned or have not responded to treatment. It works by blocking FGFR proteins involved in signals that can support cancer cell growth and survival.
What it treats
- Previously treated bile duct cancer (cholangiocarcinoma) in adults that cannot be removed by surgery and is locally advanced or has spread to other parts of the body, with an FGFR2 fusion or other rearrangement detected by an FDA-approved test. - Myeloid/lymphoid neoplasms (MLNs) in adults with an FGFR1 rearrangement that have returned after treatment or have not responded to treatment.
Safety information
Side effects
Common side effects reported with Pemazyre include high or low blood phosphate levels, hair loss, nail problems, diarrhoea, constipation, tiredness, changes in taste, nausea, vomiting, mouth sores, dry mouth, dry eyes, dry skin, reduced appetite, abdominal pain, joint pain and back pain. Other common effects reported in people with myeloid/lymphoid neoplasms with FGFR1 rearrangement include rash, anaemia, nosebleeds, pain in the arms or legs, indigestion, blurred vision, swelling of the hands or feet, and dizziness. Contact your doctor urgently if you develop blurred vision or any other changes in vision. Pemazyre can cause retinal problems, including retinal pigment epithelial detachment and fluid-related retinal detachment. Eye examinations are needed before and regularly during treatment, even if you have no visual symptoms. High blood phosphate levels require monitoring because they can lead to mineral deposits in soft tissues and calcium deposits in the skin, including a condition called calciphylaxis. Your doctor may need to interrupt, adjust or stop treatment depending on how severe or persistent this is. Pemazyre can harm an unborn baby. Contact your doctor if you are pregnant or think you may be pregnant. Patients who could conceive a child should use effective contraception.

Before taking this medicine
The FDA label lists no contraindications for Pemazyre. Important precautions include: Pemazyre can cause retinal pigment epithelial detachment, an eye problem. Eye examinations, including optical coherence tomography (OCT), are required before treatment, every 2 months for the first 6 months, and every 3 months thereafter. Any visual symptoms require an urgent eye examination. Pemazyre can raise phosphate levels in the blood, which may lead to mineral or calcium deposits in soft tissues and skin. Phosphate levels must be monitored, and treatment may need to be adjusted, paused or stopped depending on the severity and duration. Pemazyre can harm an unborn baby. Patients who can become pregnant or father a child should use effective contraception. Do not breastfeed during treatment.
Interactions
Avoid taking Pemazyre with medicines that strongly or moderately increase or block the activity of an enzyme called CYP3A. If a medicine that blocks CYP3A cannot be avoided, your prescriber will need to reduce your Pemazyre dose. Ask your doctor to check your other medicines for these interactions.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
