About Pegasys

Overview

Pegasys contains peginterferon alfa-2a and is a prescription medicine injected under the skin. It treats chronic hepatitis C in adults and children aged 5 years and older whose liver still functions adequately, usually with other hepatitis C medicines. It also treats chronic hepatitis B in certain adults whose liver still functions adequately and certain children aged 3 years and older without cirrhosis, depending on tests showing viral activity and liver inflammation. It works by activating the body’s natural antiviral immune response.

What it treats

- Chronic hepatitis C in adults with compensated liver disease, in combination with other hepatitis C medicines. Use alone is limited to adults who cannot take other hepatitis C medicines because of a contraindication or significant intolerance. - Chronic hepatitis C in children aged 5 years and older with compensated liver disease, in combination with ribavirin. - Chronic hepatitis B in adults with compensated liver disease, active viral replication and liver inflammation, whether HBeAg-positive or HBeAg-negative. - HBeAg-positive chronic hepatitis B in children aged 3 years and older without cirrhosis, with active viral replication and elevated liver enzyme ALT levels.

Safety information

Side effects

Common side effects include tiredness or weakness, fever, muscle aches and headache. These occurred in more than 40% of adults in clinical trials. Common side effects in children are similar. Contact a doctor promptly for depression or other mental health changes, a return to drug misuse, infections, or problems affecting the heart, lungs, liver, thyroid or blood sugar. Serious reactions also include reduced blood cell production, autoimmune disorders, eye problems, inflammation of the colon or pancreas, and nerve damage when used with telbivudine. Liver function can worsen in people with cirrhosis, and hepatitis may worsen during hepatitis B treatment. Combination treatment may impair growth in children. Seek immediate medical help for suicidal thoughts, a drug overdose, signs of a stroke, a severe allergic reaction, or a severe skin reaction such as Stevens–Johnson syndrome. Some reactions can be life-threatening or fatal. When used with ribavirin, treatment can cause birth defects or death of an unborn baby. A negative pregnancy test before treatment, two effective forms of contraception and monthly pregnancy tests are required.

When to call your doctor

- Seek immediate medical attention for severe or worsening depression, or thoughts of suicide. - Seek immediate medical attention for fever with signs of a serious infection. - Seek immediate medical attention for a severe skin reaction or allergic reaction.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not use Pegasys if you have: • A known allergy to alpha interferons, including Pegasys, or any of its ingredients. Reactions may include hives, swelling, narrowing of the airways, anaphylaxis or Stevens–Johnson syndrome. • Autoimmune hepatitis. • Cirrhosis with liver decompensation (a Child-Pugh score above 6, or 6 or higher if you have both chronic hepatitis C and HIV). Pegasys must not be used in newborns or infants because it contains benzyl alcohol, which can cause serious and sometimes fatal complications. When Pegasys is used with other hepatitis C antiviral medicines, their contraindications also apply. Pegasys with ribavirin must not be used by pregnant women or men whose female partners are pregnant. Ribavirin can cause birth defects and fetal death. Combination treatment requires a negative pregnancy test before treatment, two effective forms of contraception and monthly pregnancy tests. Close monitoring is needed for mental health or neurological problems, heart or blood vessel disorders, reduced blood cell production, autoimmune conditions, thyroid disorders, high blood sugar, eye problems, stroke-related disorders, lung problems, infections, colitis, pancreatitis, and serious allergic or skin reactions. These problems may require treatment changes or discontinuation. Liver disease requires close monitoring; treatment must be stopped immediately if liver decompensation occurs. Hepatitis B may worsen during treatment. Severe kidney impairment requires a prescriber-directed dose adjustment. Older adults may have more severe mental health, heart-related or flu-like reactions. Combination treatment may impair growth in children. Safety and effectiveness have not been established for chronic hepatitis C in children under 5 or chronic hepatitis B in children under 3. Use with telbivudine may cause peripheral nerve damage. Safety and effectiveness have not been established in people with hepatitis B who also have hepatitis C or HIV.

Interactions

Tell your doctor if you take theophylline, methadone, nucleoside analogues or zidovudine. Pegasys may increase theophylline levels in the blood, so your doctor should monitor these levels and may adjust your treatment. With methadone, monitoring is needed for signs of methadone toxicity. Nucleoside analogues require close monitoring for toxic effects. If these worsen, your doctor may need to reduce or stop Pegasys, ribavirin or both. Zidovudine taken with Pegasys and/or ribavirin requires monitoring for worsening low white blood cell counts (neutropenia) or low red blood cell counts (anemia).

Dosing is set by your prescriber. Each health care professional may prescribe medication differently according to a patient's medical history and condition, so follow the directions on your prescription label and speak to your doctor or pharmacist before changing anything.

Storage

Store in the original carton to protect it from light. Check the pack for whether it must be refrigerated before first use. Keep all medicines out of the sight and reach of children.
