About Paxil

Overview

Paxil contains paroxetine, a selective serotonin reuptake inhibitor (SSRI). The supplied FDA label is for Paxil CR and lists its use in adults for major depressive disorder, panic disorder, social anxiety disorder and premenstrual dysphoric disorder. Paroxetine’s exact mechanism is unknown, but it is thought to work by reducing the reuptake of serotonin by nerve cells, increasing serotonin activity in the brain.

What it treats

Used in adults to treat major depressive disorder (MDD), panic disorder, social anxiety disorder and premenstrual dysphoric disorder (PMDD).

Safety information

Side effects

Common side effects include nausea, constipation, diarrhea, dry mouth, decreased appetite, dizziness, drowsiness, difficulty sleeping, weakness, sweating, tremor and changes in vision. Sexual side effects include reduced sex drive, erection problems, abnormal ejaculation and female genital problems. Contact a doctor promptly for serious side effects, including suicidal thoughts or behaviors, allergic reactions, serotonin syndrome, unusual bleeding, mania or hypomania, seizures, angle-closure glaucoma, low blood sodium or bone fractures. Serotonin syndrome can occur when paroxetine is taken alone, and the risk increases with other medicines that affect serotonin. Bleeding risk may increase with aspirin, nonsteroidal anti-inflammatory drugs, antiplatelet medicines or anticoagulants such as warfarin. Symptoms can also occur when treatment is stopped. Paroxetine may harm an unborn baby; exposure during the first trimester is linked to an increased risk of heart malformations. Contact your doctor if you are pregnant or become pregnant during treatment.

When to call your doctor

• Seek medical attention immediately for suicidal thoughts or behavior, or worsening mental health. • Get immediate medical help if you have a seizure. • Seek immediate medical attention for unusual bleeding.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

Do not take Paxil if you: - Are taking a monoamine oxidase inhibitor (MAOI), or have stopped one within the past 14 days. This includes linezolid and intravenous methylene blue, because of the risk of serotonin syndrome. - Are taking pimozide or thioridazine, because of the risk of QT prolongation, a change in the heart’s electrical rhythm. - Have a known allergy to paroxetine or any of the medicine’s inactive ingredients. Tell your prescriber if you have bipolar disorder, a seizure disorder or untreated narrow angles in the eyes. Paroxetine may trigger mania or hypomania, requires caution in people with seizures, and may trigger angle-closure glaucoma in people with untreated narrow angles. Serotonin syndrome can occur with paroxetine alone, and the risk increases when it is taken with other medicines that affect serotonin. Bleeding risk may increase with aspirin, nonsteroidal anti-inflammatory drugs (NSAIDs), other antiplatelet medicines, warfarin or other anticoagulants. Discuss treatment with your prescriber if you are pregnant or breastfeeding. Paroxetine may harm the unborn baby, including an increased risk of heart malformations with exposure in the first trimester. Use later in pregnancy may increase the risk of serious lung circulation problems and difficulties adapting after birth. If breastfeeding, a decision is needed about whether to stop the medicine or breastfeeding, taking into account the importance of treatment to the mother.

Interactions

Tell your doctor if you take warfarin or other medicines that bind strongly to blood proteins. Side effects may need monitoring, and your doctor may need to adjust your medicines. Paroxetine can affect medicines broken down by the liver enzyme CYP2D6. Your doctor may need to adjust those medicines. If you take tamoxifen, discuss this with your doctor. The label recommends considering a different antidepressant that has little or no effect on CYP2D6.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
