About Nplate
Overview
Nplate (romiplostim) is an injectable medicine used to treat low platelet counts in adults with immune thrombocytopenia (ITP) and children aged 1 year and older who have had ITP for at least 6 months. For ITP, it is used when corticosteroids, immunoglobulins or spleen removal have not worked well enough and the condition increases the risk of bleeding. It is also indicated to increase survival in adults and children, including full-term newborns, after acute exposure to radiation that suppresses bone marrow function. It works by activating thrombopoietin receptors, which signals the body to produce more platelets.
What it treats
- Low platelet counts in adults with immune thrombocytopenia (ITP) when corticosteroids, immunoglobulins or spleen removal have not worked well enough. - Low platelet counts in children aged 1 year and older who have had ITP for at least 6 months, when those treatments have not worked well enough. - To increase survival in adults and children, including full-term newborns, after acute exposure to radiation that suppresses bone marrow function (hematopoietic syndrome of acute radiation syndrome). For ITP, use is limited to patients whose low platelet counts and clinical condition increase bleeding risk, not to bring platelet counts into the normal range. Nplate is not indicated for low platelet counts caused by myelodysplastic syndrome (MDS) or causes other than ITP.
Safety information
Side effects
Common side effects in adults include joint pain, dizziness, trouble sleeping, muscle pain, pain in the arms or legs, abdominal pain, shoulder pain, indigestion, and tingling or prickling sensations. Headache is also commonly reported. In children, common side effects include bruising, upper respiratory tract infections, and mouth or throat pain. Serious risks that warrant contacting a doctor include blood clots associated with increased platelet counts. Clots in the vein carrying blood to the liver have been reported in patients with chronic liver disease. In some patients with myelodysplastic syndromes (MDS), Nplate increases immature blood cells called blasts and the risk of progression to acute myelogenous leukemia. A severely low platelet count may persist during treatment because of antibodies that block Nplate’s effect or other causes.
Before taking this medicine
The FDA label lists no contraindications for Nplate (romiplostim). However, important cautions apply: - In some patients with myelodysplastic syndromes (MDS), Nplate increases immature blood cell (blast) counts and the risk of progression to acute myelogenous leukemia. - Increases in platelet counts with Nplate have led to blood clots. Clots in the portal vein have been reported in patients with chronic liver disease receiving Nplate. - Severely low platelet counts may persist during treatment because of antibodies that block Nplate’s activity or other causes. - Nplate may harm an unborn baby. Do not breastfeed during treatment.
Interactions
Nplate may be used with other treatments for immune thrombocytopenia (ITP), including corticosteroids, danazol, azathioprine, intravenous immunoglobulin (IVIG), and anti-D immunoglobulin. Tell your doctor if you are receiving any of these treatments.
