About NERLYNX

Overview

NERLYNX (neratinib) is a kinase inhibitor medicine supplied as tablets. It is used alone in adults with early-stage HER2-positive breast cancer as extended treatment after trastuzumab-based therapy given following surgery. It is also used with capecitabine for advanced or metastatic HER2-positive breast cancer in adults who have received at least two previous anti-HER2 treatment regimens for metastatic disease. It works by blocking the activity of proteins called EGFR, HER2 and HER4, which are involved in cell growth signals.

What it treats

- Early-stage HER2-positive breast cancer in adults, used alone as extended treatment after adjuvant trastuzumab-based therapy. - Advanced or metastatic HER2-positive breast cancer in adults, used with capecitabine after at least two previous anti-HER2 treatment regimens for metastatic disease.

Safety information

Side effects

Common side effects of NERLYNX include diarrhea, nausea, vomiting, reduced appetite and tiredness. When used alone, other common effects include stomach pain, rash, mouth sores, muscle spasms, indigestion, raised liver enzymes, nail problems, dry skin, bloating, nosebleeds, weight loss and urinary tract infections. When used with capecitabine, other common effects include constipation, weakness, weight loss, dizziness, back or joint pain, urinary tract infections, upper respiratory tract infections, bloating, reduced kidney function and muscle spasms. Contact your doctor if diarrhea occurs, especially if it is severe or persistent. It may require additional treatment, fluids and electrolytes, and your doctor may need to interrupt or stop NERLYNX. NERLYNX can also cause liver damage. Liver function blood tests are needed monthly for the first three months, then every three months during treatment, and as clinically needed. Serious liver abnormalities may require treatment to be interrupted or stopped. NERLYNX can harm an unborn baby. Use effective contraception and contact your doctor if you become pregnant during treatment.

When to call your doctor

• Seek medical attention immediately if you have severe diarrhea or diarrhea that does not go away.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

The FDA label lists no contraindications for NERLYNX. However, important precautions apply: NERLYNX can cause diarrhea. Severe or persistent diarrhea may require treatment to be paused, and some cases require permanent discontinuation. Follow your prescriber’s plan for preventing and managing diarrhea. NERLYNX can cause liver damage. Liver function tests are needed monthly for the first 3 months, then every 3 months during treatment, and as clinically indicated. Serious liver abnormalities may require treatment to be paused or permanently stopped. NERLYNX can harm an unborn baby. Use effective contraception during treatment and discuss the risk to a fetus with your prescriber. Women should not breastfeed while taking NERLYNX.

Interactions

NERLYNX should not be taken with proton pump inhibitors, a type of medicine that reduces stomach acid. Other acid-reducing medicines, including H2-receptor antagonists and antacids, require specific timing gaps from NERLYNX. Ask your doctor how to separate them. Also avoid medicines that: • Strongly inhibit the enzyme CYP3A4. • Both inhibit P-glycoprotein (P-gp) and moderately inhibit CYP3A4. • Strongly or moderately increase CYP3A4 activity. Your doctor should check whether any of your medicines fall into these groups. Some medicines transported by P-gp can cause serious side effects even with small changes in their levels. If used with NERLYNX, they require monitoring for side effects.

Dosing

Dosing is set by your prescriber. Each health care professional may prescribe medication differently according to a patient's medical history and condition, so follow the directions on your prescription label and speak to your doctor or pharmacist before changing anything.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
