About Mekinist

Overview

Mekinist tablets contain trametinib, a kinase inhibitor used to treat certain cancers with specific BRAF gene mutations. Used alone, it treats melanoma that cannot be removed by surgery or has spread, with a BRAF V600E or V600K mutation, in people who have not previously received a BRAF inhibitor. In combination with dabrafenib, it is used for certain melanomas, non-small cell lung cancers, anaplastic thyroid cancers, other solid tumours and childhood low-grade gliomas, subject to specific mutation and treatment criteria. It works by blocking MEK1 and MEK2 proteins involved in signals that promote cell growth.

What it treats

- Melanoma that cannot be removed surgically or has spread, with a BRAF V600E or V600K mutation. Used with dabrafenib, or alone in patients who have not previously received a BRAF inhibitor. - Melanoma with a BRAF V600E or V600K mutation and lymph node involvement, after complete surgical removal. Used with dabrafenib as additional treatment. - Non-small cell lung cancer that has spread, with a BRAF V600E mutation. Used with dabrafenib. - Locally advanced or spreading anaplastic thyroid cancer with a BRAF V600E mutation, when there are no satisfactory local or regional treatment options. Used with dabrafenib. - Solid tumors with a BRAF V600E mutation that cannot be removed surgically or have spread, in adults and children aged 1 year and older, after progression following previous treatment and when no satisfactory alternatives remain. Used with dabrafenib. This indication has accelerated approval; continued approval may depend on confirmatory trials. - Low-grade glioma with a BRAF V600E mutation in children aged 1 year and older who need systemic therapy. Used with dabrafenib. Mekinist is not indicated for colorectal cancer.

Safety information

Side effects

Common side effects of Mekinist (trametinib) used alone include rash, diarrhea and swelling caused by a build-up of lymph fluid (lymphedema). When Mekinist is used with dabrafenib, common side effects vary by cancer type and age group. They include fever, tiredness, nausea, vomiting, rash, chills, diarrhea, headache, muscle or joint pain, swelling, dry skin, reduced appetite, constipation, cough and bleeding. Other common effects in some groups include high blood pressure, shortness of breath, abdominal pain, acne-like skin inflammation and inflammation around the nails. Contact a doctor promptly about serious side effects or signs of them. These include: • Major bleeding. • Inflammation of the colon or a hole in the stomach or intestine. • Blood clots in deep veins or the lungs. • Heart muscle problems. • Eye problems, including blockage of a retinal vein. Any change in vision needs an eye assessment. • Lung inflammation. New or worsening unexplained breathing symptoms need prompt assessment. • Serious fever reactions when used with dabrafenib. • Severe skin reactions or associated skin infections. • High blood sugar, which requires monitoring in people with existing diabetes or high blood sugar. • Hemophagocytic lymphohistiocytosis (HLH). • New cancers, including skin cancers, when used with dabrafenib. Monitoring is needed before, during and after treatment. Mekinist can also harm an unborn baby. People who could become pregnant should use effective contraception.

When to call your doctor

- Seek medical attention immediately for unusual bleeding or severe abdominal pain. - Get urgent medical help for new or worsening unexplained shortness of breath or cough. - Contact your doctor immediately for any changes in vision. - Get urgent medical help for a high fever, with or without chills, especially if also taking dabrafenib. - Seek medical attention immediately for a severe rash.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

The FDA label lists no contraindications for Mekinist. However, it includes important warnings and precautions. Mekinist can cause serious bleeding, inflammation or perforation of the bowel, blood clots in the legs or lungs, heart muscle problems, eye problems, lung inflammation and serious skin reactions. Heart function should be checked before and during treatment. Any changes in vision require an eye assessment. Patients with diabetes or high blood sugar need blood glucose monitoring. When used with dabrafenib, Mekinist can cause new cancers, including skin cancers, and serious fever reactions. Monitoring for new cancers is needed before, during and after treatment. The label also warns about hemophagocytic lymphohistiocytosis (HLH); treatment must be interrupted if it is suspected and discontinued if confirmed. Mekinist can harm an unborn baby. Females who can become pregnant should use effective contraception. Do not breastfeed during treatment. Mekinist may impair fertility; discuss pregnancy planning and prevention with your prescriber.

Interactions

When Mekinist is used with dabrafenib, additional interaction risks may apply. Ask your doctor to review the dabrafenib prescribing information for risks relevant to your treatment.
