About Macrobid
Overview
Macrobid contains nitrofurantoin, an antibacterial medicine used for acute, uncomplicated bladder infections caused by susceptible Escherichia coli or Staphylococcus saprophyticus bacteria. It is not intended to treat kidney infections or abscesses around the kidneys. It works by disrupting several essential processes inside bacteria, killing them in the urine.
What it treats
Acute uncomplicated urinary tract infections (acute cystitis) caused by susceptible strains of Escherichia coli or Staphylococcus saprophyticus.
Safety information
Side effects
The most frequently reported side effects in Macrobid clinical trials were nausea (8%), headache (6%) and gas (1.5%). Less frequent effects included diarrhea, indigestion, stomach pain, constipation, vomiting, dizziness, drowsiness, itching and hives. Contact a doctor promptly if you develop: • Cough, shortness of breath, chest pain, fever or chills. These may indicate a lung reaction. Acute reactions usually occur within the first week. Chronic reactions generally occur after six months or longer of continuous treatment and can cause permanent lung damage. • Symptoms of nerve damage, which can be severe or irreversible. Fatal cases have been reported. • Severe or persistent diarrhea during or after treatment, which may indicate inflammation of the colon. • Yellowing of the skin or eyes. Rare liver reactions include hepatitis and destruction of liver tissue. • Changes in vision, confusion or other unusual mental changes. Seek immediate medical attention for swelling of the face or throat, difficulty breathing, or a severe rash with blistering or peeling. Serious allergic reactions, including anaphylaxis, and severe skin reactions, including Stevens–Johnson syndrome, have been reported. Other reported serious effects include inflammation of the pancreas, increased pressure around the brain, and blood disorders such as anemia or reduced white blood cells or platelets. Rarely, an abnormal form of hemoglobin can cause bluish skin. New infections caused by resistant bacteria or fungi can also occur.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take Macrobid if you: • Produce no urine or very little urine, or have significantly impaired kidney function (creatinine clearance below 60 mL/min or a clinically significant rise in serum creatinine). Reduced drug clearance increases the risk of toxicity. • Are 38–42 weeks pregnant, in labor or delivery, or about to go into labor, because of the risk of hemolytic anemia (breakdown of red blood cells). • Have previously had cholestatic jaundice (jaundice caused by impaired bile flow) or liver dysfunction linked to nitrofurantoin. • Have a known allergy to nitrofurantoin. Macrobid must not be given to babies under one month of age because of the risk of hemolytic anemia.
Interactions
Tell your doctor if you take antacids containing magnesium trisilicate, as they can reduce how much and how quickly nitrofurantoin is absorbed. Probenecid and sulfinpyrazone can raise nitrofurantoin levels in the blood, increasing the risk of toxicity, while lowering levels in the urine, which may make it less effective against urinary tract infections.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
