About Leucovorin
Overview
Leucovorin is a form of reduced folate supplied as tablets taken by mouth. It is used in adults to reduce the toxic effects of methotrexate when the body clears it too slowly, or after an overdose of folic acid antagonists or dihydrofolate reductase (DHFR) inhibitors. It also treats cerebral folate transport deficiency in adults and children with a confirmed variant in the FOLR1 gene. It works by providing an active form of folate that can counter the effects of folate-blocking medicines and has been observed to increase active folate levels in people with FOLR1-related deficiency.
What it treats
- To reduce methotrexate toxicity in adults whose bodies do not clear methotrexate properly. - To reduce toxicity after an overdose of folic acid antagonists or dihydrofolate reductase (DHFR) inhibitors in adults. - To treat cerebral folate transport deficiency in adults and children with a confirmed variant in the folate receptor 1 gene (FOLR1-CFTD).
Safety information
Side effects
Reported side effects include itching, rash, shaking chills and changes in body temperature. Their frequency cannot be reliably estimated from the available reports, so it is not known which are common. Contact a doctor immediately if you develop difficulty breathing, hives or signs of an allergic reaction. Severe allergic reactions, including anaphylaxis, have been reported. Leucovorin may need to be withheld or permanently stopped depending on the severity of the reaction. Safety information for oral leucovorin in treating FOLR1-CFTD is limited and based on published case reports.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.
Before taking this medicine
Do not take leucovorin if you have had an allergic reaction to leucovorin (folinic acid), levoleucovorin, folic acid, or any tablet ingredient, as follows: - For treatment of toxicity from folic acid antagonists or DHFR inhibitors: a history of a severe allergic reaction. - For treatment of FOLR1-CFTD: a history of any allergic reaction. Allergic reactions, including hives and anaphylaxis, have been reported with leucovorin. Treatment may need to be withheld or permanently stopped depending on the severity of the reaction and the condition being treated.
Interactions
Leucovorin has important interactions with the following medicines: • Antiepileptic medicines, including phenobarbital, phenytoin and primidone: closer monitoring for seizures is needed. High doses of folic acid can reduce the effectiveness of these medicines and increase seizures. It is not known whether leucovorin (folinic acid) has the same effect. Certain antiepileptic medicines may also reduce leucovorin’s effectiveness. • Trimethoprim-sulfamethoxazole: use with leucovorin should be avoided because leucovorin can reduce its effectiveness. This combination has been linked to increased treatment failure and deaths in patients with HIV being treated for Pneumocystis jirovecii pneumonia. • Fluorouracil: leucovorin may increase its toxic effects. Deaths from severe inflammation of the intestines, diarrhea and dehydration have been reported in older patients receiving both medicines. Some also had fever and low levels of certain white blood cells.

Storage
Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
