About Lenvima

Overview

Lenvima (lenvatinib) is a prescription medicine supplied as capsules, used for adults with progressive differentiated thyroid cancer that has returned locally or spread and no longer responds to radioactive iodine, and as a first treatment for hepatocellular carcinoma, a liver cancer that cannot be removed by surgery. For adults with advanced kidney cancer, it is used with pembrolizumab as a first treatment or with everolimus after one previous treatment targeting blood vessel growth. It is also used with pembrolizumab for advanced endometrial cancer that has progressed after previous systemic treatment when curative surgery or radiation is not an option, and testing with an FDA-approved test shows the cancer is mismatch repair proficient (pMMR) or not microsatellite instability-high (MSI-H). It works by blocking proteins called kinases that are involved in cancer growth and the formation of blood vessels that supply tumours.

What it treats

- Differentiated thyroid cancer in adults that is progressing, has returned locally or spread, and no longer responds to radioactive iodine. - Advanced renal cell carcinoma (kidney cancer) in adults, with pembrolizumab as first-line treatment, or with everolimus after one prior treatment targeting blood vessel growth. - Hepatocellular carcinoma (liver cancer) that cannot be surgically removed, as first-line treatment. - Advanced endometrial cancer, with pembrolizumab, when an FDA-approved test shows it is mismatch repair proficient (pMMR) or not microsatellite instability-high (MSI-H), it has progressed after prior systemic therapy, and curative surgery or radiation is not an option.

Safety information

Side effects

Common side effects vary depending on the condition being treated and whether Lenvima is taken with pembrolizumab or everolimus. They include tiredness, diarrhea, high blood pressure, muscle or joint pain, reduced appetite, weight loss, nausea, vomiting, mouth sores or inflammation, abdominal pain, headache, voice changes, protein in the urine, and redness, swelling or pain on the palms and soles. Other common effects with some treatments include an underactive thyroid, rash, constipation, bleeding, cough, shortness of breath, swelling of the arms or legs, and urinary tract infections. Liver damage and sudden kidney injury are also listed among common reactions with Lenvima and pembrolizumab in kidney cancer. Serious side effects that warrant prompt contact with a doctor include: • Severe high blood pressure, heart problems, blood clots in arteries, or changes in the heart’s electrical rhythm called QT prolongation. • Liver damage or failure, kidney damage or failure, or significant protein loss in the urine. • Severe or recurring diarrhea. • Abnormal connections between organs or tissues, called fistulas, or a hole in the stomach or intestinal wall. • Bleeding, low blood calcium, or thyroid problems. • A serious brain condition called reversible posterior leukoencephalopathy syndrome (RPLS). • Poor wound healing or damage to the jawbone, called osteonecrosis. Some serious effects require blood tests, urine tests or other monitoring to detect. Your doctor may need to pause or stop treatment depending on the reaction and its severity. Lenvima can also harm an unborn baby; effective contraception is advised for those who could become pregnant.

When to call your doctor

- Seek medical attention immediately for severe or recurring diarrhea. - Get immediate medical help for unusual bleeding. - Seek urgent medical attention for severe abdominal pain. - Get immediate medical help for difficulty breathing.

If symptoms are severe or getting worse quickly, seek emergency care rather than waiting.

Before taking this medicine

The FDA label lists no formal contraindications for Lenvima. However, it has important warnings and precautions. Careful assessment and monitoring are needed for high blood pressure, heart dysfunction, blood clots in arteries, liver or kidney problems, protein in the urine, severe or recurring diarrhea, abnormal openings between organs (fistulas), or a hole in the stomach or intestine. Other concerns include heart rhythm changes called QT prolongation, electrolyte abnormalities, low blood calcium, bleeding, thyroid dysfunction, and a brain condition called reversible posterior leukoencephalopathy syndrome (RPLS). Serious complications may require your prescriber to interrupt or stop treatment. Blood pressure should be controlled before treatment. Blood pressure, liver function, urine protein, electrolytes, calcium and thyroid function need monitoring. Lenvima can impair wound healing and cause jawbone damage. Tell your care team about planned surgery or invasive dental procedures. Treatment must be interrupted around surgery, and preventive dental care should be considered before starting. Lenvima can harm an unborn baby. Those who could become pregnant should use effective contraception. Do not breastfeed during treatment.

Storage

Store below 25°C in a dry place, in the original pack, away from direct light and moisture. Do not keep it in a bathroom. Keep all medicines out of the sight and reach of children.
